Investigate The Learning Curve Of Laparoscopic Portal Vein Reconstruction

Objective:To further research and develop the laparoscopic surgery technique,investigate the learning curve of laparoscopic portal vein reconstruction.Methods:Review and analysis the 40 cases of laparoscopic portal vein reconstruction experiment that were performed by a single group of surgeons from September 2016 to March 2018.The 40 cases were sequentially divided into four groups(groups A,B,C,and D,10 in each).The operating time,intraoperative complications and efficacy of anastomoses were compared among the 4 groups.Results:All the 40 cases operation was successfully finished.There were significant difference existed in operating time,intraoperative complications and efficacy of anastomoses among the four groups.The operating time in groups A(55.5±5.6)min and B(47.1±3.3)min were significantly longer than that in groups C(38.5±2.4)min and D(36.1±2.2)min,(P < 0.05).The experimental pig's intraoperative mortality decreased from 90%(9/10)in group A and 70%(7/10)in group B to 20%(2/10)in group C and 0%(0/10)in group D,(P < 0.05).The efficacy of anastomoses have turned a better trend compared group A to group B,group B to group C and group C to group D,respectively.But they have no statistical significances(P > 0.05).The efficacy of anastomoses in group D were significant better than in group A and B(P < 0.05).The average operation frequency is 2.3 cases per month.Conclusion:Laparoscopic portal vein reconstruction experiment is feasible in well equipment and technically developed medical center.The learning curve of laparoscopic portal vein reconstruction is about 30 cases.
