Part one Ultrasound-guided Percutaneous Microwave Ablation for Hepatocellular Carcinoma: Study on the Results and Prognostic Factors Objective: The aim of this study was to evaluate the clinical outcomes of ultrasound-guided percutaneous microwave ablation(US-guided PMWA)for the treatment of hepatocellular carcinoma(HCC)with analysis of prognostic factors.Materials and Methods: The treatment and survival parameters of 433 patients with HCC(?10cm),who met the inclusion criteria and had received US-guided PMWA in Renji Hospital from July 2010 to November 2014,were retrospectively analyzed.Imaging examination(contrast-enhanced CT or MR)and tumor markers(AFP and CA199)one month after MWA were used to evaluate the efficacy of US-guided PMWA.SPSS software was used to perform all statistical analysis.Results: The initial complete ablation(CA)rate was 94.9%(411/433).Twenty-two patients with incomplete ablation received repeat PMWA,and the total complete ablation rate was up to 98.6%(427/433).Multiple tumor number,tumor >5cm in diameter,and higher serum AFP level(>20ng/ml)were significant unfavorable prognosticators of progression-free survival(PFS).The cumulative 1-,2-,and 3-year overall survival rates were 83.5%,66.1% and 58.7%,respectively(median: 43 months).Tumor >5cm in diameter and serum AFP > 400ng/ml)were significant unfavorable prognosticators of overall survival(OS).Conclusions: PMWA is well tolerated in HCC patients and capable of offering high CA rate.Tumor number,tumor size and AFP level were significant prognosticators of patients' RFS whereas tumor size and AFP level were significant prognosticators of OS?Part two Vitro study of thermal ablation combing GPC-3 CART Therapy Background and Objective: Coagulation necrosis range of tumors were often limited during the ablation process due to the heat-sink effect and tumor diameter which render the local recurrence of tumor after the ablation therapy.The aim of this study was to detect the GPC-3 expression on HCC cell lines after heated at 56 ? and then evaluate the treatment effect combing the GPC-3 CART Cells.Materials and methods: HUH7 and PLC/PRF5 cells were incubated in the water bath for 1min at 56? and then back into the 37? incubator?The expression of GPC-3 protein were analyzed by western-blot,Flow Cytometry at 6h,12 h,24h,48 h,72h ?The m RNA level of GPC-3 were analyzed by RT-PCR at the same time point.To determine whether GPC-3 CART cells were responsive to the tumor cells heated at 56?.We then put them together for a 18-h cytotoxicity? Results: The expression of GPC-3 protein were significantly reduced at 6 hour after thermal stimulation and then slowly recovered at 6-72 h.Differently,the m RNA level of GPC-3 were rapid increased after heated,peaked at 12 h,reduced after that and then keep stable at 48-72 h.Cytotoxicity assay showed that the HCC cells lines were liable to be killed by GPC-3 CART cells after heated at 56??
