Objective: to investigate Clinical study of modified pedicle screw system in the treatment of anterior pelvic ring fractures.Methods: a retrospective analysis in October 2016-June 2018,16 patients with pelvic fractures,16 regular line 4 pieces of pedicle screws before pelvic ring fractures percutaneous internal fixation(ASIF,anterior subcutaneous internal fixator)treatment,including 9 cases of male and female 7 cases;Age 21-68 years old,average age 40.5 years old.Cause of injury: 7 cases of car accident injury,4 cases of high falling injury,5 cases of crush injury.According to the classification of OA/OTA,there were 3 cases of type 61-a2.3,1 case of type 61-b1.1,2 cases of type 61-b2.1,2 cases of type 61-b2.2,2 cases of type 61-b2.3,1 case of type 61-b3.1,1 case of type 61-b3.2,2 cases of type 61-b3.3,and 2 cases of type 61-c1.2.The operative time,intraoperative blood loss,wound healing,fracture healing,postoperative functional recovery and postoperative complications were recorded.10 patients with pelvic complex unstable,including 2 cases of sacroiliac self-contained nail fixed sacral fractures associated with dislocation,4 cases of steel plate fixed iliac fracture,4 cases of unilateral sacroiliac joint dislocation,spinal and pelvic fixation of pelvic fixation after first,then subcutaneous anterior ring fixation,the other 6 cases after the pelvis stable complexes,intraoperative direct anterior ring subcutaneous internal fixation.Results: all the 16 patients were followed up for 6-14 months,with an average of 10 months.The intraoperative blood loss of ASIF was(40-5.2)ml,the operative time was(58-7.9)min,and the postoperative hospitalization time was(4.5-2.5)d.There was no wound infection,bone nonunion,or bone nonunion.Screws became loose in 5 cases and the fracture healed.Conclusion: the application of four pedicle screw system ASIF in the treatment of bilateral pubic ramus fractures with or without pubic symphysis separation of the anterior pelvic ring has the advantages of less blood loss,shorter operation time,stronger stability,higher fracture healing rate and fewer complications,which is worth learning and promoting.
