Objective: We sought to analyze the factors influencing the first compliance time of patients with acute pulmonary embolism after oral administration of warfarin.Methods:Patients with acute pulmonary embolism who were admitted to the second hospital of Hebei medical university for the first time between October 2016 to January 2019 for the first time were collected.According to the first compliance time(TIA),patients were divided into group I:?7d;group II: 7d<TTA?14d;group III:14d<TIA?28d;group IV:TIA>28d.The study included the first anticoagulant treatment index,anticoagulant safety index and efficiency index of warfarin.Results:A total of 201 patients with acute pulmonary embolism were included.Univariate statistical analysis showed that there were statistically significant differences among the initial dose of 2.5mg group,3.0mg group and 5.0mg group in the first standard time of warfarin(P<0.05).There were statistically significant differences in the first standard time of warfarin between the normal thyroid function group,hyperthyroidism group and hypothyroidism group(P<0.05).According to different BMI in China,the patients were divided into normal group,super-recombinant group,grade I obesity group and grade II obesity group.The differences among the four groups in the first standard time of warfarin were statistically significant(P<0.05).The patients were divided into two groups according to whether they had heart failure or not,and the difference between the two groups in the first standard time of warfarin was statistically significant(P<0.05).There was no significant difference between gender and age in the first standard time of warfarin(P<0.05).In summary,initial dose,thyroid function,BMI and heart failure were the influencing factors of warfarin's first standard time.Sequential Logistic regression analysis later showed that initial dose,thyroid function,BMI and heart failure were independent influencing factors of warfarin first standard time.Conclusion:1.Univariate statistical analysis showed that initial dose,thyroid function,BMI and heart failure were the influencing factors ofwarfarin's first complian-ce time.2.Ordered Logistic regression analysis showed that initial dose,thyroid function,BMI and heart failure were independent influencing factors of warfarin's first standard time.High initial dose,high thyroid function,low BMI and heart failure can shorten the time for warfarin to reach the standard for the first time.3.The standard time of warfarin is affected by multiple factors,and various factors should be comprehensively evaluated in clinical work to achieve safe effective and rapid standard.
