Objective:The aim of this study was to investigate the expression of platelet(PLT)and lactate dehydrogenase(LDH)in patients with small cell lung cancer(SCLC)and its clinical significance,so as to guide clinical,evaluate prognosis and improve patient survival.Methods:The study collect clinical and follow-up data of 122 SCLC patients who were diagnosed by pathology in Hebei General Hospital from Jan.2013 to Dec.2017.Another 40 healthy examiners were collected as controls.PLT was measured by abbott automatic blood cell analyzer,with the normal range was(150-350)×10~9/L.LDH was determined by AU5800 Automatic enzyme label detection,the normal range was 120-250IU/L.The relationship of PLT,LDH levels and the response to chemotherapy was analyzed.Another important purpose of this paper was exploring the correlation between PLT and LDH levels and prognosis of SCLC.Results:1.The pretreatment levels of PLT and LDH in SCLC patientsThe PLT and LDH levels of SCLC patients were[290(174.25)]×10~9/L and[264.5(106)]IU/L,respectively.In the healthy control group,PLT and LDH levels were[240.5(106)]×10~9/L and[165.25(31.02)]IU/L,respectively.PLT and LDH levels in SCLC patients were significantly higher than those in the healthy control group(Z1=-3.476,P1=0.000;Z2=-6.118,P2=0.000).2.The relationship between PLT and LDH levels and clinicopathological features in SCLC patientsThe level of PLT was significantly correlated with age,tumor size,distant metastasis,clinical stage,LDH level and NSE level(P<0.05).LDH level was associated with tumor size,pleural effusion,distant metastasis,clinical stage,PLT level and NSE level(P<0.05).PLT and LDH levels were positively correlated.3.The relationship between the levels of PLT,LDH and the short term therapeutic effect.After 2 cycles of chemotherapy,9(7.4%)cases of 122 SCLC patients achieved complete response(CR),81(66.4%)patients achieved partial response(PR),and the overall remission rate(CR+PR)was 73.8%.Among them,there were 75 cases with PLT<350×10~9/L,61(81.3%)cases achieved CR+PR,47 cases with PLT?350×10~9/L,and 29 cases(61.7%)achieved CR+PR.There was statistical difference(P=0.016).57 cases with LDH<250IU/L,48(84.2%)cases achieved CR+PR;65 cases with LDH?250IU/L,and 42 cases(64.6%)achieved CR+PR.There was statistical difference(P=0.014).4.The relationship between PLT,LDH levels and prognosisThe median survival time of all patients was 14 months,and 1-year,2-year and 3-year survival rates were 57.5%,24.2%and 11.0%,respectively.The median survival time of 58 LD-SCLC patients and 64 ED-SCLC patients were 17 months and 10 months,respectively.And the 1-year,2-year and3-year survival rates were 74.0%,35.2%,16.4%and 41.7%,13.9%,6.0%,respectively.There were significant statistical differences between the two groups(P=0.000).The median survival time of 75 patients with PLT<350×10~9/L and 47patients with PLT?350×10~9/L were 15.0 months and 12.0 months,respectively.The 1-,2-,and 3-year survival rates were 63.4%,30.8%,16.3%and 47.8%,13.8%,2.8%.Statistical differences were observed(P=0.009).The median survival time of 57 patients with LDH<250IU/L group and65 cases of LDH?250IU/L group were 16.0 months and 10.0 months respectively;the 1-,2-,and 3-year survival rates were 70.9%,34.4%,18.2%,and 44.0%,12.7%,2.5%,Statistical differences were observed(P=0.000).Univariate analysis showed that KPS score,tumor size,pleural effusion,distant metastasis,clinical stage,PLT count,LDH level,Neuro-specific Enolase(NSE)level and Hemoglobin(HB)level were correlated with prognosis(P<0.05);COX regression analysis showed KPS score,tumor size,clinical stage,LDH level,HB level were the independent prognostic factors of survival for SCLC patients.Conclusions:1.The pretreatment PLT and LDH levels were significantly higher than that of normal people.PLT and LDH levels were positively correlated;2.PLT and LDH levels were positively correlated with tumor burden,distant metastasis,clinical stage and NSE level.3.High PLT and LDH levels were negatively correlated with short-term efficacy and prognosis.
