Objective:In this study,the changes of hemoglobin and oxidative stress state after oral and intravenous iorn supplementation in patients with renal anemia of maintenance hemodialysis were observed.The purpose of this study was to investigate the efficacy and safety of rHuEPO in combination with different routes of iron therapy for renal anemia in patients with maintenance hemodialysis due to renal anemia through oral and intravenous routes of iron-dependent hemoglobin and inflammatory changes.Methods : Forty patients with anemia who received maintenance hemodialysis from January 2016 to December 2016 in our hospital dialysis center were randomly divided into intravenous group(intravenous iron sucrose 100 mg once daily)and oral group(oral supplementation of dextran Iron 100 mg,2 time / d).The levels of hemoglobin(Hb)and hematocrit(Hct)before and after 4,8,and 12 weeks of treatment were observed in both groups.The changes of iron status before and after 4,8,and 12 weeks after treatment were observed in both groups(SF),TSAT,biochemical indicators: BUN,Cr,ALT,AST,and the correlation of oxidative stress in vivo Indicators: changes of superoxide dismutase(SOD),malondialdehyde(MDA)and so on.Results: The intravenous Hb,Hct,SF,and TSAT levels were increased after intravenous administration of iron for 4 weeks(P<0.05).The oral administration of Hct and Hb increased after oral administration of iron for 8 weeks(P<0.05),and there was a statistically significant difference compared with that before iron supplementation.There was no statistically significant difference in oral iron supplementation between SF and TSAT before and after oral treatment.After 12 weeks of iron supplementation,the Hb,Hct,SF and TSAT correlations in the intravenous group were significantly higher than those in the oral group,and the difference was statistically significant(P<0.05).There was no significant difference in the average rHuEPO dose between the oral group and the intravenous group.After 12 weeks of treatment,plasma SOD levels in the intravenous group were significantly lower than before treatment(P<0.05),and plasma MDA levels were significantly higher than before treatment(P<0.05);There was no statistically significant difference in oxidative stress parameters in the oral group compared with that before the experiment.Conclusions:1.Intravenous administration of sucrose iron to treat renal anemia in maintenance hemodialysis patients is superior to oral administration of dextran iron.2.Intravenous route iron supplementation can exacerbate oxidative stress in maintenance hemodialysis patients.
