The Value Of CK18 Fragment Detection In The Diagnosis Of Chronic Hepatitis B Liver Fibrosis

Objective: To observe the correlation between the expression of serum of CK18 and the progression of hepatitis b and the degree of hepatic fibrosis,and to explore the value of CK18 serological expression in the diagnosis of chronic hepatitis b liver fibrosis.Methods : Selection of Q ingdao municipal hospital digestive diseases and infections,Qingdao sixth people's hospital,a total of 252 patients with liver of hepatitis b viral hepatitis,diagnosis accord in 2000,the first infectious diseases and parasitic epidemiology branch of C hinese medical association,liver disease,joint of the revised "viral hepatitis preve ntion and control program" in viral hepatitis diagnosis standard,on the basis of the viral hepatitis prevention scheme,diagnostic criteria for clinical classification: The experimental group was divided into acute hepatitis,mild chronic hepatitis,chronic hepatitis,moderate group and severe chronic hepatitis,cirrhosis group,46 cases of healthy physical examination and take the same period,the experimental group and healthy controls was detected in serum CK18 segment(and M65 level decomposition,and make comparison between groups,for statistical analysis.Results: 1.There was no significant difference in age and sex between the experimental group and the control group(P <0.05).2.With the progress of chronic hepatitis,ALT and AST levels were grad ually increasing(P < 0.05).2.With the aggravation of the disease,the white ball ratio decreased gradually,and the differences among the groups were statistically significant(P < 0.05).3.The contents of serum CK18 fragment M30 and M65 were successively increased in the healthy group,acute hepatitis group,chronic hepatitis group and cirrhosis group,and the difference was statistically significant(P < 0.05).4.The healthy group,acute hepatitis,chronic hepatitis group and hepatocirrhosis group were compared between two groups: the healthy control group,chronic hepatitis group and hepatocirrhosis group between the two comparison,CK18 fragment(content differences between groups was statistically significant(P < 0.05).There was no statistically significant difference in the M30 content between the acute hepatitis group and the healthy control group,the chronic hepatitis severe group and the liver cirrhosis group(P BBB 0.05).C irrhosis of the liver and chronic hepatitis group between,between health and severe chronic hepatitis group and healthy group and between groups,acute hepatitis,cirrhosis and severe chronic hepatitis,acute hepatitis group and hepatocirrhosis group between segments of CK18 M65 content difference was statistically significant(P < 0.05).There was no statistically significant difference in the content of M65 in CK18 fragments(P <0.05).There was no statistically significant difference in the content of M65 in CK18 fragments(P <0.05).There was no significant difference in the M65 content between the mild group of chronic hepatitis and the moderate group of chronic hepatitis,the mild group of chronic hepatitis and the severe group of chronic hepatitis(P < 0.05).Conclusion: The results of this study showed that with the progression of chronic hepatitis b,the contents of M30 and M65 of serum CK18 were gradually increased,suggesting that CK18 fragments had clinical value in the progression of chronic hepatitis b.According to previous studies on the liver fibrosis of CK18 and chronic liver disease,it is speculated that CK18 has clinical value in the progression of chronic hepatitis b liver fibrosis.Disease progression in chronic hepatitis b including CK18 fragment(content as well as an increase in early and there are st atistically significant,speculation CK18 fragment(may be more sensitive to early diagnosis of chronic hepatitis b liver fibrosis,CK18 fragment M65 content rise late in the treatment of chronic hepatitis b disease progression is more apparent,speculated that serum CK18 fragment M65 may tip is more specificity of liver fibrosis.
