Objective:To observe the efficacy and safety of salvianolic acid for injection in patients with acute ischemic stroke,and to explore new targets and new directions for drug intervention in patients with acute ischemic stroke,so as to provide drug selection directions for good therapeutic effects in patients with acute ischemic stroke.Methods:From February 2017 to March 2018,153 patients with acute ischemic stroke admitted to our hospital were divided into observation group(n=70)and control group(n=83).The patients who received conventional medication were the control group.The observation group was treated with salvianolic acid for injection on the basis of conventional drugs.Both groups of patients were given medication for 14 days.The changes of sCD40 L,ET-1,VEGF-A,sICAM-1,CRP and platelet function indexes were monitored before treatment and 14 days after treatment.The adverse reactions during the medication were observed.After 14 days of treatment,the degree of reduction and improvement of NIHSS scores in the two groups were analyzed and compared between groups.The clinical outcomes after 90 days of follow-up were evaluated using the modified Rankin scale.Multivariate logistic regression analysis was used to determine the independent prognostic factors for patients with acute ischemic stroke who were followed up for 90 days.Results:(1)After treatment for 14 days,ET-1,VEGF-A,sCD40 L,CRP and sICAM-1 were significantly lower in the two groups than in the control group,and the levels of ET-1,slCAM-1 and CRP in the observation group were significantly lower than those in the control group.All were statistically significant(P < 0.05).(2)After treatment for 14 days,the platelet aggregation rate of the two groups was significantly lower than that before the administration,and the decrease of platelet aggregation rate in the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).(3)There were no significant differences in liver function,renal function and myocardial enzyme related test indexes between the two groups after 14 days of treatment(P>0.05).(4)After 14 days of treatment,the NIHSS scores of the two groups were lower than those before treatment,but the NIHSS scores of the observation group were significantly lower,and the improvement rate of NIHSS scores was more significant(P<0.05).(5)After the follow-up of 90 days after discharge,the mRS score of the observation group was significantly lower,and the proportion of the good prognosis was higher,while the proportion of poor prognosis was lower,the difference was statistically significant(P<0.05).(6)Using the good prognosis as a dependent variable and including its possible influencing factors as independent variables,a multivariate logistic regression analysis was performed.The results showed that the use of salvianolic acid for injection and the improvement of neurological function were acute ischemic Independent predictors of good prognosis after stroke in 90 days.Conclusion:The salvianolic acid for injection can effectively reduce the levels of sCD40 L,CRP,slCAM-1,VEGF-A,ET-1 and platelet aggregation,and exert anti-inflammatory,reduce ischemia-reperfusion injury,protect the blood-brain barrier,inhibit platelet aggregation,protect nerve cells,and improves the effectiveness of clinical treatment of acute ischemic stroke,and it is safe and worth promoting.
