| Objective:To analyze the risk factors of anastomotic fistula after radical resection of esophageal cancer in our hospital,to provide theoretical basis for reducing the incidence of anastomotic fistula and improving the prognosis of anastomotic fistula,and to summarize the experience of diagnosis and treatment.Methods:The clinical data of 201 patients with esophageal cancer who underwent surgical treatment in our hospital from June 2012 to June 2018 were retrospectively analyzed.Several factors,including preoperative albumin level,diabetes mellitus,pneumonia and anastomotic location,were selected to analyze the risk factors of anastomotic leakage after esophageal cancer surgery.Multivariate logistic regression was used to analyze the independent risk factors.Results:There were 201 patients in this group,44 cases of anastomotic leakage occurred after operation,the incidence was 21.9%(44/201),1 case died,the fatality rate was 0.5%(1/201).There were 172 cases of cervical anastomosis,42 cases of anastomotic leakage after operation,the incidence was 24.4%(42/172),29 cases of intrathoracic anastomosis,2 cases of anastomotic leakage,the incidence was 6.9%(2/29).Univariate analysis showed that preoperative diabetes mellitus(P=0.001),preoperative albumin level(P=0.000),postoperative pneumonia(P=0.000)and anastomotic location(P=0.035),atotal of four factors P<0.05,with statistical significance.Multivariate logistic regression analysis showed that preoperative diabetes mellitus(P=0.010,OR=3.972),post-operative pneumonia(P=0.000,OR=10.675),anastomotic location(P=0.013,OR=0.115),preoperative albumin level(P=0.000,OR=0.383)were independent risk factors for anastomotic leakage after esophageal cancer surgery.Conclusion:This study confirmed once again that diabetes mellitus,pneumonia after operation,anastomotic location and preoperative albumin level may be risk factors for anastomotic leakage after esophageal cancer surgery.It should be prevented.Early diagnosis and timely and correct treatment are the key to cure anastomotic leakage after esophageal cancer operation. |
