Objective:To study the clinical characteristics and laboratory characteristics of MPO-ANCA positive and PR3-ANCA positive patients,in order to determine whether the two were different.To further study the clinical characteristics of the two groups of patients with renal involvement,and explore the factors affecting the prognosis of AAV patients.To provide more basis for optimizing the treatment plan,reducing the recurrence rate and improving the survival rate of AAV patients.Methods:According to the ANCA serotypes,8 patients were in the PR3-ANCA positive group and 34 in the MPO-ANCA positive group.To study the clinical characteristics and differences between the two groups,and the two groups of patients were further analyzed according to the presence or absence of renal involvement.And to explore the risk factors and prognosis of recurrence in AAV patients.Statistical analysis was performed using independent sample t test,chi-square test,single factor correlation analysis,logistic regression model,and COX regression.Results:(1)Patients in the PR3-ANCA positive group were younger(P=0.007),and were more likely to have ENT symptoms(P=0.001),the difference was statistically significant.(2)In the PR3-ANCA positive renal involvement group,BVAS>15 was the most common,proteinuria was positive,CRP was higher,the difference was statistically significant(P<0.05).(3)BVAS was associated with eye,ENT,nervous system,and death,and the difference was statistically significant.(4)BVAS > 15 points is a risk factor for recurrence.(5)There was no significant difference in survival rate between the two groups,but the survival rate of the two groups was similar before half a year ago,while the PR3-ANCA positive group maintained a survival rate of about 87.5%,while the MPO-ANCA positive group continued to decline.Conclusion:(1)The PR3-ANCA positive group is younger and prone to ENT symptoms.(2)BVAS is significantly associated with involvement in the eye,ENT,nervous system,and death.(3)BVAS>15 points at diagnosis is an independent risk factor for recurrence.(4)There was no significant difference in survival rate between the MPO-ANCA-positive and PR3-ANCA-positive groups,but the survival rate was similar within 6 months after onset,and the survival rate of the MPO-ANCA-positive group continued to decrease. |
