Objective:Leukoaraiosis(LA)is frequently reported on brain imaging.It is a group of clinical syndromes that is characterized by spotted or patchy ischemic changes in the deep subcortical and periventricular white matters.The purpose of this study was to investigate the effects of high-density lipoprotein and other clinically relevant factors on leukoaraiosis in acute ischemic stroke patients.It aimed to improve the prognosis of cerebral infarction by controlling the risk factors of leukoaraiosis to delay or reduce the occurrence of leukoaraiosis.Method:778 patients with acute ischemic stroke aged 41-93 years old who underwent head MRI in the Second Affiliated Hospital of Soochow University from January 2014 to October 2018 were selected as subjects.According to the Fazekas score,they were divided into non-LA group and LA group,and according to the severity of LA,they were divided into non-LA group?mild LA group?moderate LA group and severe LA group.According to the location of the disease,they were divided into periventricualr leukoaraiosis group and subcortical leukoaraiosis group.The clinical data of the grouped patients were analyzed.According to one year follow-up,the subjects were divided into regular medication group and irregular medication group to analyze the relationship between drugs and changes in LA severity.Result:LA group compared with non-LA group,age,history of hypertension,history of stroke,admission systolic blood pressure,calcium count,high-density lipoprotein,homocysteine,WLR were independent risk factors for LA.Age,history of hypertension,history of stroke,admission diastolic blood pressure,admission diastolic blood pressure,HDL and WLR were independent risk factors for different severity of LA.The history of hypertension in patients with Periventricular leukoaraiosis was 5.245 times higher than that in patients with deep subcortical leukoaraiosis.The difference was statistically significant.Increased HDL concentration was positively correlated with increased Fazekas score.After one year of follow-up,taking statins regularly was positively associated with increased severity of LA;patients older than 65 years of age,the increased Fazekas scores of taking statins regularly was 3.017 times that of taking statins irregularly.Conclusion:HDL was an independent risk factor for LA in patients with acute ischemic stroke.High-level HDL(HDL>l,33mmol/l)was positively correlated with LA severity.More than 65 years old patients who took statins regularly got higher Fazekas score.
