Objective:The aim of the study is to explore the difference of testosterone level among Chinese POI women,normal women of pre-menopause and post-menopause,to guide the treatment and health care of POI women.Methods:The premature ovarian insufficiency patients who were admitted to the outpatient and inpatient department of Women's Hospital of Zhejiang University Medical College during December 2018 with complete medical history and assessment data of ovarian reproductive and endocrine functions,were selected as the research group(POI group).Another selection of the same period in the hospital physical examination or outpatient gynecology patients,ovarian function assessment of normal or natural menopause of the female population as the pre-final menstrual period control(Ctrl-Pre-FMP)and the post-final menstrual period control(Ctrl-Post-FMP).Exclusion criteria were:a history of using hormone drugs within 3 months,chromosomal abnormalities,thyroid and adrenal diseases,polycystic ovarian syndrome(hyperandrogenism),uterine and ovarian tumors,and a history of invasive ovarian operation or pelvic radiotherapy.The serum testosterone level of the three groups was compared with each other by the sub-groups according to age(<30 years old group;30?<35 years old group;35?<40 years old group;40?45years old group),serum FSH level(?15 IU/L group;>25 IU/L group;>40 IU/L group)and serum AMH level(<2.0 ng/mL;<0.5 ng/mL).Results:1.85 Chinese women were included in the POI group,212 in the Ctrl-Pre-FMP group and 131 in the Ctrl-Post-FMP group.There was no statistically significant difference in age composition between the POI group and Ctrl-Pre-FMP group.2.Serum testosterone level(0.52±0.34 nmol/L)in POI group was significantly lower than that in Ctrl-Pre-FMP group(0.81±0.33 umol/L),and significantly higher than that in Ctrl-Post-FMP group(0.41±0.36 nmol/L),with statistically significant differences(P<0.05).3.POI patients in different serum FSH levels:(15 IU/L group;>25 IU/L group;>40 IU/L).The corresponding serum testosterone level of>40 IU/L was significantly lower than that of the Ctrl-Pre-FMP group(0.81±0.33 nmol/L).The serum testosterone level(0.55±0.35 nmol/L)in the serum FSH level 40 IU/L group was significantly higher than that in the Ctrl-Post-FMP group(FSH>40 IU/L,T 0.38±0.34 nmol/L),with statistically significant differences(P<0.05).4.POI patients in different serum AMH levels(<2.0 ng/mL),the corresponding serum testosterone level was also significantly lower than that of the Ctrl-Pre-FMP group(AMH?2.0 ng/mL,T 0.83±0.35 nmol/L)5 and the difference was statistically significant(P<0.05).5.POI patients of different ages(18?<30 years old group;30?<35 years old group;35?<40 years old group;The corresponding serum testosterone levels of the 40?45 years old group were all lower than those of the Ctrl-Pre-FMP group of the corresponding age group,and the corresponding differences were statistically significant before the age of 40(P<0.05).Conclusion:1.Serum testosterone level of Chinese POI patients was lower than that of women of the same reproductive age,and higher than that of postmenopausal women2.The significant decrease of serum testosterone level in POI patients has appeared in the early stage(subclinical stage)?3.Corresponding health management and care should be initiated and implemented in the early POI stage(subclinical stage).
