Objective:Through the study of the therapeutic effect and relapse of narrow-band ultraviolet B combined with tripterygium wilfordiipoly glycosides on patients with Chronic actinic dermatitis(CAD),the levels of IFN-γand IL-12 in serum of CAD patientswere detected to explore its clinical therapeutic effectin the treatment of CAD.Methods:60 patients with CAD were selected from the dermatology clinic and ward of the First Affiliated Hospital of Hebei North University from October 2016 to September 2018.The normal control group was 60cases of health check-up in the same period.CAD patients were randomly divided into three groups:A(tripterygium wilfordiipoly glycoside treatment group),B(narrow-band UVB treatment group)and C(narrow-band UVB+tripterygium wilfordiipoly glycoside treatment group).The initial dose of phototherapy was 70%of The minimum erythema dose of narrow-band ultraviolet B(NB-UVB-MED),the subsequent dose was determined by the patient’s tolerance to NB-UVB,patients in groups B and C were treated with phototherapy 28 times,while patients in groups A and C were given tripterygium wilfordiipoly glycoside tablets 20mg 3 times per day for 8 weeks after oral treatment,and the recurrence was observed after follow-up for 8 weeks after the treatment of the three groups.NB-UVB-MED(NB-UVB-MED Measuring Instrument and NB-UVB Radiotherapy Cabin Phototherapy Instrument of Waldmann Company in Germany was used),γ-IFN,IL-12 levels(Enzyme Linked Immunosorbent Assay method,ELIAS)and Eczema Area and Severity Index(EASI)scores before and after treatment were measured for CAD patients.The minimum erythema dose of NB-UVB-MED andγ-IFN,IL-12levelswere measured for normal control group.SPSS 25.0 was used to analyze the data.Results:(1)the value of NB-UVB-MED(226.67±142.46)mJ/cm~2 in CAD patients was significantly lower than that in normal people(626.67±207.38)mJ/cm~2,P<0.01,the difference was statistically significant.The NB-UVB-MED values of typeⅢskin in CAD patients and normal people were significantly lower than those of typeⅣskin,P<0.01,with statistically significant difference.The comparison of NB-UVB-MED values between different genders,P>0.05,showed no significant difference.(2)Among CAD patients,20 were included in group A,21 in group B and 19 in group C.Compared with the general data among the three CAD groups,P>0.05,the difference was not statistically significant.EASI scores of CAD groups decreased after treatment compared with before treatment,P<0.05,and the difference was statistically significant.The curative effect of group C is higher than that of group B and group A,P<0.05,the difference is statistically significant.The effective rate of group C is higher than that of group B and group A,P<0.017,the difference is statistically significant,the effective rate of group A and group B is compared,P>0.017,the difference is not statistically significant.(3)Among CAD patients,the number of follow-up patients in group A was 19,group B was 19,and group C was 18 8 weeks after treatment.The recurrence rate of group C is lower than that of group A and group B,P<0.017,the difference is statistically significant,the recurrence rate of group A and group B is compared,P>0.017,the difference is not statistically significant.(4)the levels of IFN-γand IL-12 in serum of CAD patients were significantly higher than those of normal people,P<0.01,the difference was statistically significant.The levels of IFN-γand IL-12 in CAD three groups after treatment were significantly lower than before treatment,P<0.01,the difference was statistically significant.Conclusion:1.Narrow band UVB,tripterygium glycosides and combined therapy are effective for CAD patients,of which combined therapy has the best effect,lowest recurrence rate.2.The content of IFN-γand IL-12 in CAD patients is higher than that in normal people.Narrow-band UVB,tripterygium glycosides and their combination therapy may be reduced the levels of IFN-γand IL-12 in CAD patients. |
