Objective:(1)Establish self-management mode of internal hemodialysis patients to increase patient compliance and internal self-care ability(2)Strengthen the self-management behavior of dialysis patients in internal hemorrhoids,promote self-protection of internal hemorrhoids,improve the quality of internal hemorrhoids,and improve the quality of dialysis.Methods:Randomly selected 90 patients with internal hemorrhoids who were undergoing maintenance hemodialysis at the Blood Purification Center of Wuhan Union Medical College Hospital were randomly divided into two groups.The control group received nursing care according to conventional internal hemorrhoid nursing measures.The intervention group also increased self-management in addition to conventional internal hemorrhoid nursing.The education of mode of thinking and the intensity of self-management compliance are strengthened.Based on the differences in the quality of intrinsic quality of the two groups,it is assessed whether self-management has a relevant influence on the quality of internal hemorrhoids.Before the experimental intervention,the purpose,significance,methods,and other relevant contents of this study were performed by the investigators to the staff of the blood purification center who participated in this study.In the identified intervention group and control group,the work that was trained in this study was performed.The staff collected information in the form of patient talks,questionnaires,etc.The quality assessment of internal hemorrhoids was evaluated by the nurses of the blood purification center,the puncture of the fixed nurses for the corresponding patients,information collection,internal assessment,etc.Control group: Internal pelvic care includes knowledge related to guilt;internal self-assessment of guilt quality(self-perception of tremor,pulsation,blood flow shock,abnormal self-awareness of internal hemorrhoids);self-assessment of guilt;Missionary;Inner Caloric Therapy(Non-Heat Kangpu Infrared Treatment Instrument and Ordinary Hot Pack,etc.);Medication at Intraorbital Department(Hirudoid,Lidocaine Rub,Potato Wet Dressing,etc.)Mission;Regular Intrinsic Lid Test(Ultrasonic Assessment)General health education content.Intervention group: Routine internal caries nursing were the same as the control group.Introduction to the concept of self-management model;education based on 20 behavioral items in the hemodialysis patient self-management scale;re-education by responsible nurses on self-management content;family support.Results:(1)The variance of self-management total scores of hemodialysis patients showed that the time effect was statistically significant(F=216.703,P<0.01)at three time points before intervention,one month after intervention,and three months after intervention.There was a statistically significant difference between the groups;the intervention group was >control group(F=191.097,P<0.00l);there was an interaction between intervention effect and time effect(F=161.459,P<0.00).?(2)Repeated measurement of variance of self-efficacy of hemodialysis patients before and after intervention.Results of analysis of variance before and after intervention,1 month after intervention,and 3 months after intervention,the time effect was statistically significant(F=319.939),P<0.001);statistical significance between groups,intervention sister> control group(F=53.904,P<0.001);interaction between group effects and time effects(F=206.752,P<0.001).(3)Analysis of changes in the quality of internal hemorrhoids was performed using repeated measures analysis of variance.The results showed that there were statistically significant time effects at the three time points before intervention,one month after intervention,and three months after intervention(F=149.864).(P<0.001);There was a statistically significant difference between groups,intervention group>control group(F=23.797,P<0.001);interaction effect between intervention effect and time effect(F=126.115,P less than 0.001).The establishment of self-management model for patients with internal fistula has effectively improved the self-management ability of regular hemodialysis patients.In the daily life of individuals,the patients with self-management ability of internal fistula daily nursing,self-diagnosis of internal fistula.The use of internal fistula,medical communication and other aspects have been improved.Conclusion:This study shows that improving the self-care ability of patients can improve the skills and knowledge level of self-care diseases and improve the self-care responsibility,so as to better manage the disease,improve the patient's health status,and improve the patient's quality of life.After the intervention group intervention,the total score of self-care ability was higher than that of the control group(p<0.05),and the difference between the intervention group and the intervention group was statistically significant(p<0.05).It shows that the self-efficacy intervention can improve the self-care ability of patients more than conventional interventions.The study also shows that the self-management model has a positive effect on the quality of patients' internal hemorrhoids.Before the intervention,there was no significant difference in the scores of the dimensions of the self-management scale and the quality assessment of internal hemorrhoids in the hemodialysis patients of the control group and the intervention group,indicating that the two groups were well balanced and comparable.After intervention,the scores of the dimensions of the self-management scale of the hemodialysis patients in the control group and the quality assessment of internal hemorrhoids were significantly higher than those of the control group.After the establishment of the self-care model for the observation group,counseling and supportive care for possible complications and how to prevent complications during dialysis were found.The results showed that the incidence of thrombosis,bleeding,and infection was significant in the observation group.The incidence of complications such as thrombosis,infection,and hemorrhage was significantly lower in the patients than in the control group.The difference was statistically significant.The quality of internal hemorrhoids directly influences whether the patient's hemodialysis can be carried out smoothly.The patient with built-in self-management mode has higher quality of internal hemorrhoids and fewer complications.It can be easily punctured and treated during hemodialysis because of the vascular access.Ensure that the blood flow rate can reach the theoretical optimal speed of 250ml/h,and the dialysis is more complete.In the course of this study,patients in the intervention group were more satisfied with the nursing staff in the department than before.Compared with the control group,the degree of recognition of staff in the blood evolution center was improved.This is related to the increase in communication between nurses and patients during the study.During the intervention process,the infusion of the new concept of self-management led to an increase in the professionalism of the staff of the blood evolution center.During this process,the patient's identity and trust with the nurse of the blood purification center increased.In the course of daily self-care,the nursing staff gave the patient more psychological support and initiative,which effectively stimulated the patient's confidence in their own disease control.The relationship between nurses and patients continued to progress throughout the entire study process,creating a good nursing environment and promoting patient satisfaction.In summary,patients apply the concept of self-management to improve their self-care capabilities and level of self-care and self-care,and maintain and improve the quality of self-care.Through the use of good internal hemorrhoids to ensure the smooth and effective conduct of hemodialysis to reduce the related hemodialysis complications,improve dialysis and quality of life,and ultimately extend their lives.
