Outcomes Of Long Segment And Short Segment Fixation In The Treatment Of Degenerative Scoliosis:A Systematic Review And Meta-analysis

Objective: To evaluate the clinical efficacy of long segment and short segment fixation in the treatment of degenerative scoliosis.Methods: Chinese journal full-text database,Chinese biomedical literature database,Chinese scientific journal database,Wanfang database were searched by Chinese keywords such as “short segment,long segment,adult degenerative scoliosis,degenerative lumbar scoliosis”.Chinese database;search for PubMed,Cochrance library,Embase database with "short fusion,long fusion,adult degenerative scoliosis,Degenerative lumbar scoliosis".Use the above keywords to find randomized controlled trials or cohort studies using short or long segments to treat degenerative scoliosis.Extract uniform evaluation indicators such as preoperative postoperative coronal cobb angle,lumbar lordosis angle,operation time,Oswestry disability index(ODI),etc.,and then use RevMan 5.3 and statistical software for data analysis.get conclusion.Results: Eight articles were included,six in Chinese and two in English,with a total of 571 cases.There were 302 cases with short segments and 269 cases with long segments.The short segment group is superior to the long segment group in terms of operation time,while the long segment group surgery is superior to the short segment group in the correction of the coronal Cobb angle and lumbar lordosis angle in the lumbar spine.The difference was statistically significant(P>0.05).Conclusion: Long segment fixed short segment fusion fixation has better curative effect in correcting coronal cobb angle and postoperative lumbar lordosis angle.However,in terms of operation time,short segment fusion fixation has obvious advantages.There was no statistical difference in postoperative ODI scores and VAS scores.
