Objecyive The aim of this study was to investigate the changes of serum fibroblast factor-23(FGF-23)levels in patients with type 2 diabetes mellitus with diabetic kidney disaese(DKD)and their correlation.Methods From October 2017 to May 2018,82 patients(45 males and 37 females)with type 2 diabetes in the Endocrinology department of the first affiliated hospital of Bengbu Medical College were selected.The subjects were divided into two groups according to the diagnostic criteria of WHO diabetes mellitus in 1999 and K/DOQI recommendations in 2007:Type 2 diabetes was accompanied by DKD(group A 1)and type 2 diabetes without DKD(group A 2).Subjects in each group was collected general information and laboratory specimens in the morning of the second day of admission before treatment.General biochemical parameters of patients were recorded.All subjects were examined by color Doppler ultrasound of bilateral carotid artery and lower extremity artery,including right internal carotid artery medial thickness(RICA IMT),left common carotid artery thickness(LICA IMT).Serum FGF-23 levels were measured by bridging ABC-ELISA sandwich method in patients with type 2 diabetes mellitus(T2DM)Results(1)The serum FGF23 concentration in group A was significantly higher than that in group B(t=2.123,P<0.05),The SCr,BUN,UA,P,CRP,HbA1c,24h-ALB,PTH,RICA IMT,LICA IMT of group A was higher than that of group B,and the difference was statistically significant(t=-2.544?-2.237?-2.453?2.737?-2.949?-2.143?-2.769?-2.645?-2.508?-2.495,P<0.05).The 24h-Alb,25-OH-D3 of group A was lower than that of group B(t=2.893?0.819,P<0.05).(2)The prevalence of carotid atherosclerosis in group A was higher than that in group B(?~2=4.717,P<0.05).The prevalence of lower extremity atherosclerosis in A group(75.0%)was also higher than that in B group(?~2=4.438,P<0.05).(3)Multiple linear regression analysis showed that serum FGF23 was affected by BMI,SCr,UA,LPa,CRP,P in A1 patients(standardized regression coefficient?=0.183?0.545?-0.301?0.242?0.346?0.317,P<0.05).Conclusions The serum FGF23 level in type 2 diabetes mellitus with DKD was significantly higher than that in type 2 diabetes mellitus without DKD.
