ObjectivesUsing independent component analysis to explore aberrant function connectivity within default-mode network(DMN)in patients with end-stage renal disease(ESRD)with long-term regular hemodialysis.Furthermore,pearson correlation analysis was used to analyze the impact of clinial basic data and laboratory blood biochemical indicators on the brain network of ESRD hemodialysis patients.Using Independent Component Analysis(ICA)to study abnormal changes in the functional connectivity of the default mode network(DMN)in patients with end-stage renal disease(ESRD)with long-term regular hemodialysis The relationship with cognitive function levels,and further explore the impact of clinical basic data and laboratory blood biochemical indicators on the brain network of ESRD hemodialysis patients.MethodsAccording to the inclusion criteria of this experimental study,24 ESRD hemodialysis patients were selected,and 23 healthy controls were recruited.The age,sex and years of education were matched.The MRI data were recorded by a Siemens Trio 3.0 T MR scanner,include T2WI?DWI?3D-T1 WI whole brain structure image and resting state functional MRI imaging scan.Before the brain MRI,the cognitive function level of all subjects was assessed using the Montreal cognitive assessment(Mo CA)and the Mini-mental state examination(MMSE).The Blood biochemical indicators of ESRD patients were detected,such as red blood cell count,hemoglobin content,serum creatinine,urea nitrogen,blood calcium,blood potassium,blood sugar,triglycerides,total cholesterol,etc.First,a one-sample t-test was used to separate and extract the functional connectivity of the default model brain network between the two groups of subjects.Then,the default model brain network of the two groups of subjects obtained was double-sampled.The test was performed to obtain the differential brain regions between the two groups,and statistical analysis was performed using SPSS22.0 statistical software.Finally,Pearson correlation analysis was conducted between the FC values of the difference brain areas between the two groups and the scores of neuropsychiatric intelligence scale,general clinical basic data and laboratory blood biochemical index parameters.Results1.Adopts double sample t test in patients with ESRD hemodialysis group compared with healthy controls the default mode of brain network,compared with healthy controls,patients with ESRD group on the right side of the Medial prefrontal cortex(MPFC)of FC value increased significantly,after the Posterior cingulate cortex(PCC)/Precuneus(PCu)FC value significantly lower,and were statistically significant(P < 0.05).2.There were statistically significant differences in cognitive function scores between the two groups(P<0.05).Differences between the two groups of FC in value and cognitive function score,chronic kidney disease progression,dialysis duration and laboratory of fixed number of year of the blood biochemical indexes of correlation analysis,the precuneus/cingulate FC value after the cholesterol levels of patients with ESRD hemodialysis group showed a negative correlation(r = 0.471,p = 0.03),the other clinical parameters had no significant correlation.Conclusions1.The default mode brain network of ESRD hemodialysis patients has abnormal functional connectivity changes,which may be the neuropathological basis of their cognitive dysfunction.2.Abnormal brain areas in the default mode brain network of ESRD hemodialysis patients are mainly concentrated in the medial prefrontal lobe and precuneus/posterior cingulate gyrus,which are closely related to the memory of human brain,suggesting that the main early cognitive dysfunction may be mainly manifested as memory decline.3.Resting state functional mri is expected to be a sensitive and effective means to detect cognitive dysfunction in ESRD hemodialysis patients.The early detection and diagnosis of cognitive dysfunction are conducive to the prognosis and quality of life of ESRD patients.
