Objectives:To investigate the prevalence,genotype and related factors of BK polyomavirus(BKV)and JC polyomavirus(JCV)in HIV-1 infected patients.To analyze the JCV NCCR and VP1 coding sequence of patients concurrent AIDS and central nervous system diseases(CNSD).Methods:1.Urine samples from 137 human immunodeficiency virus 1(HIV-1)infected patients in Zhejiang Province in China were tested for the presence of BKV and JCV DNA.The detected VP 1 sequences were aligned and analysed using BioEdit and MEGA software.2.Real-time PCR was performed on CSF,plasma,PBMC,and urine samples obtained from 57 patients with concurrent AIDS and central nervous system lesions.The JCV VP1 coding region and NCCR were sequenced and analyzed.Results:(1).Among urine samples from HIV-1-infected patients,64.2%were positive for BKV DNA(88/137),67.2%were positive for JCPyV DNA(92/137),and 44.5%were positive for both BKV and JCV(61/137).The genotypes of BKV isolated were IV and I;the genotype 7 of JCV were detected,among which 45.5%(15/33)were subtype 7A,30.3%(10/33)were 7B,and 24.2%(8/33)were 7C.Nucleotide mutations found in seven urine samples were not associated with amino acid substitution.Statistical analysis showed that there was no difference in gender,age,CD4 count and HAART among groups of polyomavirus infection.Univariate analysis and Binary logistic analysis showed tuberculosis co-infection was an independent risk factor for BKV infection(P=0.044).(2).The positive rates of JCV in the CSF,plasma,PBMC,and urine of the AIDS patients with CNSD were 4/53(7.5%),2/51(3.9%),2/49(4.1%)and 17/25(68.0%),respectively.In AIDS patients with PML,NCCR rearrangement was found in samples CZS-4,CZS-9,CZP-9,and CZN-15.These mutated sequences have not been reported before and were identified for the first time in this study.Furthermore,amino acid substitutions of VP1 sequences were also found in these samples.Conclusion:1.In the urine of HIV-1-infected people,there is a high infection rate of polyomavirus.Tuberculosis co-infection is an independent risk factor for BKV infection.The genotypes of BKV isolated were ? and ?;the genotype of JCV isolated was type 7.2.We found that urine samples have the highest JCV prevalence in AIDS patients with CNSD.In CSF,plasma or PBMC sample,AIDS patients with PML have both NCCR recombination and VP1 amino acid substitutions.
