Objective:To compare the clinical effect of thyroid lobectomy and partial thyroidectomy in benign thyroid nodules which have obvious clinical symptoms.Methods:84 patients with benign thyroid nodules which have obvious clinical symptoms treated from April 2014 to April 2016 in our hospital were selected.According to the results of preoperative ultrasound examination and puncture pathology,combined with patients'wishes,the subjects were assigned to study group and control group,42 cases each group.The patients received the thyroid lobectomy and partial thyroidectomy,respectively.The clinical effect was compared.Results:The operation time of the study group was longer than that of the control group,and the amount of bleeding and postoperative drainage were less than those of the control group(P<0.05).There was no significant difference in ambulation time and length of stay(P>0.05).The incidence of complications in the study group was 11.9%,and in the control group was 21.43%.The incidence of complications in the study group was significantly lower than that in the control group(P<0.05).There was no significant difference in thyroid hormone level,nodule diameter and volume between the two groups before treatment(P>0.05).There was no significant difference in serum total triiodothyronine(TT3)and serum thyroid hormone(TT4)levels between the two groups after treatment(P>0.05),but serum thyroid stimulating hormone(TSH)levels in the study group were higher than those in the control group(P>0.05).<0.05)the level of thyroid peroxidase(TPO)was lower than that of the control group(P<0.05).After treatment,pain in the study group was not significantly different from that in the control group,and the result was not statistically significant(P>0.05).There were 11 cases of paraffin pathological nodular goiter,28 cases of thyroid adenoma and 3 cases of papillary thyroid carcinoma in the study group.In control group,there were 10 cases of nodular goiter,30 cases of thyroid adenoma,and 2 cases of papillary thyroid carcinoma.In the control group,2 cases recurred within 1 year,accounting for 4.76%,5 cases recurred within 2 years,accounting for 11.9%,the recurrence risk was significantly higher than that in the study group.Conclusion:Patients with benign thyroid nodules with clinical symptoms usually have larger nodal volume.The operation time of lobectomy is slightly longer than that of partial thyroidectomy,but it has fewer complications and lower recurrence rate,which reduces the difficulty and risk of reoperation due to residual recurrence or postoperative pathological malignancy.The therapeutic effect is better.
