Objcet:In this research,we intented to study the prevalence of HPV infection and distribution characteristics of HPV subtypes in The people’s Hospital of Xiangxi prefecture.And we analyzed distribution of HPV subtypes in different grades of cervical disease.Then we explore the infection characteristics of HPV and related factors of Xiangxi prefecture.It will provide scientific basis for the prevention of cervical cancer in Xiangxi prefecture.Methods:1.From October 2017 to October 2018,clinical data of6324 patients received by gynecology outpatient departmengt of The people’s Hospital of Xiangxi prefecture were collected.Retrospective study was conducted to analyze the overall prevalence of HPV infection,the prevalence of HPV subtypes infection.And the prevalence of HPV infection in differengt ages and subtypes were compared.2.According to the pathological diagnosis,85 normal cases and 404cervical disease case which was diagnosed in the hospital were selceted.The difference of prevelence of HPV infection between normal cases and cervical disease cases and distribution of HPV subtypes in different grades of cervical disease were compared.3.497 patients who underwent HPV typing test in The people’s Hospital of Xiangxi prefecture during the same period and voluntarily accepted the questionnaire survey were selected.We collect the possible related factors of HPV infection such as race,usual residence,education level,occupation,reproductive status,previous history of gynecology and gynecological examination.SPSS 22.0 statistical software was used to conduct single Chi-square test and multi-factor non-conditional Logistic regression analysis,and then HPV infection related factors in the region were obtained.Results:Among the 6324 patients treatded in the outpatient department of The people’s Hospital of Xiangxi prefecture,1257 cases of HPV positive and 5067 cases of HPV negative were detected,and the overall prevalence of HPV was 19.88%.The infection rate of high-risk HPV was 17.66%,and low-risk was 2.21%.The single subtype infection rate was 15.34%,and mixed subtype infection rate was 4.54%.The most common subtypes were HPV52(4.87%),58(3.23%),16(3.19%),53(2.17%),39(1.88%).2.The infection rate of HPV was different in different age groups,which were 38.10%in the age group less than or equal to 20 years old,20.22%in the age group of 21-30 years old,15.38%in the age group of 31-40 years old,19.01%in the age group of 41-50years old,24.65%in the age group of 51-60 years old,29.04%in the age group of more than 60 years old.There were significant difference in the HPV prevalence in each age group(x~2=73.747,p=0.000).Two peaks of HPV infection were appeared at the age of less than or equal to 20 years old and more than 60 years old.The low peak of HPV infection were appeared at the age of 31-40 years old.3.The infection rate of was12.94%in normal cervical tissue,53.03%in LSIL,97.44%in HSIL and98.5%in cervical cancer.There were significant difference in the HPV infection rate between the normal cervical and the lesion cervical group(x~2=221.876,p=0.000).Among the LSIL group,the most common subtypes were HPV52,58,16,18,39.In HSIL group,The most common subtypes were HPV16,52,58,33,39,6(tied for fifth).The most common subtypes in cervical cancer were HPV16,18,33,58,52,68.The infection rate of HPV16 gradually increased with the lesion severity,which was significant difference in each group(x~2=80.867,p=0.000).4.In the single Chi-square test:race,usual residence,education level,occupation,the number of induced abortions,contraceptive method,family tumor history,and heard cervical cancer were not related to HPV infection(p>0.05).Age,menopause,delivery times,age of first delivery,previous history of gynecology,gynecological examination,heard HPV were related to HPV infection(p<0.05).In the multi-factor non-conditional Logistic regression analysis:early age of first delivery were risk factors of HPV infection(OR>1),no gynecological diease,heard HPV were protective factors of HPV infection(OR<1).Conclusions:1.HPV prevelance in Xiangxi prefecture has its own regional characteristics in terms of infection rate and subtypes,The prevention and treatment of HPV 52,58 cannot be ignored.2.HPV infection in Xiangxi prefecture is mainly of single subtypes,and the infection trend is related to age.3.HPV infection in HSIL and cervical cancer in Xiangxi prefecture is mainly of subtype HPV16,it is necessary to take more prudent and active treatment measures for HPV16.3.Gynecological inflammatory disease within half a year,never heard HPV,early age of first delivery are risk factors for HPV infection in Xiangxi prefecture.Taking targeted measures for these factors has positive significance for prevention and management HPV infection in Xiangxi prefecture. |
