Object:The purpose of this study was to explore the application of quantitative management model of multi-form exercise therapy in gestational diabetes mellitus.And also to evaluate the effects of exercise self-efficacy,exercise compliance,blood glucose control,and maternal and child delivery outcomes during pregnancy.It provides a theoretical basis for the improvement of the existing health education model of exercise therapy and the establishment of future exercise prescriptionsMethodsA randomized controlled design was used.Through reviewing the literature,we understand the current status of exercise therapy for gestational diabetes mellitus(GDM),and combine with international guidelines and norms related to physical activity during pregnancy,a set of multi-disciplinary members including diabetes specialist nurses,senior midwifery nurses,obstetricians,nutritionists,psychological counselors and sports rehabilitation specialists jointly developed a set of multivariate quantitative exercise intervention management models.According to the inclusion and exclusion criteria,150 GDM patients who received regular obstetric examination in our hospital from January to July 2018 and intend to give birth in our hospital and volunteer to participate in the study were selected.Participants informed consent.All patients were assessed by general information questionnaire,the pregnancy exercise self-efficacy Scale(P-ESES)and international physical activity questionnaire(IPAQ)before intervention,and fasting blood glucose(FBG)and postprandial 2 hours blood glucose(2hPG)levels were measured.The patients were divided into control group(75 cases)and experimental group(75 cases)by random data table method.Patients in the control group were given routine pregnant exercise health education mode,that is,pregnant women received pregnant women's school exercise related courses until the end of childbirth.On this basis,patients in the experimental group combined with multiple quantitative exercise intervention management model,that is,to guide different forms of exercise to complete the international(ACOG)recommended moderate intensity physical activity(daily cumulative time of at least 30 minutes,weekly time of not less than 150 minutes)until the end of childbirth.After 4 weeks of intervention,the two groups were evaluated by P-ESES and IPAQ scales,and the levels of FBG and 2hPG were also measured.To compare and analyze whether there are differences between the two groups in pregnancy outcomes,such as weight gain during pregnancy,mode of delivery,complications of mother and child.All data were processed and analyzed by SPSS 19.0 statistical software package.RssultsAfter excluding 12 parturients who had dropped out of the study,delivered out of the hospital and failed to follow up,138 patients were included in the analysis,namely 68 in the experimental group and 70 in the control group.The results showed that there was no significant difference between the experimental group and the control group in the total score of exercise self-efficacy,exercise therapy level,,fasting blood glucose and 2-hour postprandial blood glucose before intervention(p>0.05).Four weeks after the intervention,the total score of P-ESES and the coincidence rate of exercise therapy level in the experimental group were significantly higher than those in the control group(p<0.05).And the scores of overcoming motor obstacle,emotional obstacle and support obstacle in the P-ESES scale in the experimental group were significantly improved after intervention(p>0.05).The fasting blood glucose and 2-hour postprandial blood glucose of the experimental group and the control group were improved after intervention,but the fasting blood glucose and 2-hour postprandial blood glucose control levels of the experimental group were significantly better than those of the control group(p<0.05).Comparing the maternal and neonatal outcomes of the two groups,the coincidence rate of weight gain during pregnancy was higher in the experimental group than in the control group,but the rate of macrosomia delivery and Cesarean section in the experimental group were lower than that in the control group(p<0.05).The incidence of neonatal hypoglycemia,neonatal asphyxia,pregnancy-induced hypertension,excessive amniotic fluid,premature rupture of membranes,premature delivery and postpartum hemorrhage had no significant difference between the two groups(p>0.05).ConclusionsQuantitative management model of multi-form exercise therapy can enhance the exercise self-efficacy,improve exercise compliance,reduce blood sugar levels,improve the quality of life during pregnancy,and reduce the occurrence of cesarean section and maternal and fetal pregnancy complications,which is conducive to promoting maternal and fetal health.
