Objective:To explore the clinical characteristics of adverse outcome after intravenous thrombolysis in patients with baseline NIHSS score of 0-5 points.Methods:Patients with baseline NIHSS score of 0-5 and a pre-mRS score of 0 were enrolled in the Department of Neurology,First Affiliated Hospital of Soochow University from March 2017 to January 2019,after the exclusion of absolute taboos.The patients treated with intravenous thrombolysis were divided into good and poor outcome group according to the mRS score?1 or>1 24 hours and 14 days after onset.The baseline indexes?risk factors?imaging characteristics were compared between the two groups at different timepoints analyzed by univariate and multivariate analysis.Results:The percentage of patients with poor 24-hour outcome was higher than that of 14-day group(43%vs 30%).OTT>3h(p=0.004,OR=3.926,95%CI 1.565-9.846)and NIHSS score(p=0.045,OR=1.417,95%CI 1.007-1.987)were risk factors for adverse outcomes at 24-hour group.OTT>3h(p=0.029,OR=2.963,95%CI 1.117-7.858),age(p=0.014,OR=1.06,95%CI 1.012-1.110),CTA-Mismatch-(p=0.021,OR:1.476,95%CI 1.061-2.054)were risk factors for poor outcome at 14-days group.A higher rate of motor function deterioration was found in patients with poor outcomes in the CTA-Mismatch-subgroup(p<0.001).Conclusion:1?The higher baseline NIHSS and the longer onset to treatment time are related to the poor short outcome.2?CTA-mismatch-is a dependent risk factor of 14-day poor outcome.We should pay more attention to the deterioration of motor symptoms in these patients and early rehabilitation may be more important.3.In clinical practice,imaging features should be combined with the time window for better choice of active treatment.
