OBJECTIVE: This study investigated the expression of Tspan5 in the eutopic and ectopic endometrium of patients with moderate to severe endometriosis,to explore its role in the occurrence and development of endometriosis,and its possible mechanism of affecting fertility in endometriosis patients.Provide new ideas for the diagnosis and treatment of patients with endometriosis-associated infertility.METHODS: The research group selected patients who were treated in our hospital’s Reproductive Medicine Center and Gynecology Department,from February 2018 to November 2018,18 patients with endometriosis were diagnosed as having ovarian chocolate cysts and were diagnosed by stage III and IV.The endometriosis fertility index score was based on the surgical status and medical history.In the control group,patients with non-endometriosis infertility caused by tube factors were selected for laparoscopic surgery.One of the endometrium was taken for pathological examination to confirm the staging,and the other was used to detect the content of Tspan5 and OPN in endometriosis,ectopic endometrium and normal endometrium of endometriosis by Real-time PCR.RESULTS: The expression of Tspan5 mRNA and OPN mRNA in eutopic endometrium and ectopic endometrium of EMs was significantly higher than that of control group(P<0.05).The expression of Tspan5 mRNA in the ectopic endometrium was also significantly higher than that in the eutopic endometrium.The expression was also significantly higher than the eutopic endometrium(P<0.05).The expression level of Tspan5 mRNA and OPN mRNA in the normal endometrial secretory phase was significantly higher than that in the proliferative group(P<0.05).There was no significant difference in the expression of Tspan5 mRNA between the proliferative and secretory phases ofendometriosis(P>0.05),and there was no significant difference in the expression of corresponding ectopic endometrium(P>0.05).The expression of OPN mRNA in the endometrium of the endometriosis patients was significantly higher than that in the proliferative phase(P<0.05),and the expression in the ectopic endometrium of the secretory phase was also higher than that in the proliferative phase(P<0.05).There was no significant correlation between the expression levels of Tspan5 and OPN and the concentration of serum CA125 in endometriosis and ectopic endometrial tissues.In the control group,the expressions of Tspan5 and OPN were positively correlated with serum E2 and P(P<0.05),while in the endometriosis group,the expression of Tspan5 and OPN were not significantly correlated with serum E2 and P levels.Sexual correlation(P>0.05).The average EFI of endometriosis patients was(5.72 ± 1.99)points.In the endometriosis and ectopic endometrial tissues,the expression level of Tspan5 was negatively correlated with EFI(P<0.05).There was no significant correlation between the expression level of OPN and EFI in endometriosis of endometriosis(P>0.05).In the endometrial tissue of endometriosis,there was no significant correlation between the expression level of Tspan5 and the expression of OPN(r=0.02,P>0.05).In the endometriosis of endometriosis,the expression level of Tspan5 was positively correlated with the expression of OPN(r=0.646,P<0.05).CONCLUSION: This study found that the high expression of Tspan5 in the endometrium with endometriosis may be one of the factors that promote the development of endometriosis.Abnormally high expression and periodic loss of Tspan5 in the eutopic endometrium of patients with EMs may affect endometrial receptivity.Tspan5 has certain advantages in assessing the reproductive potential of endometriosis and predicting reproductive outcomes.
