Elevated Serum CCN3 Levels In Newly Diagnosed Type 2 Diabetes Patients Are Associated With Obesity,Insulin Resistance And Inflammation

Objective This study measured the level of serum CCN3 in newly diagnosed type 2 diabetic patients and explored the correlation between serum levels and various metabolic parameters in humans.Methods According to the diagnostic criteria formulated by WHO in 1999,a total of 219 newly diagnosed type 2 diabetes patients(nT2DM)who visited the outpatient department of The First Affiliated Hospital of University of South China from April 2016 to September 2017 were recruited as the experimental group,and 205 healthy subjects with age and gender matching during the same period were selected as the control subjects.All subjects' basic information was recorded by questionnaire survey.Fasting blood glucose,fasting insulin,fasting c-peptide,triglyceride,total cholesterol,low density lipoprotein-cholesterol,high density lipoprotein-cholesterol,glycosylated hemoglobin,hypersensitive c-reactive protein and other biochemical indexes were determined for all subjects.BMI,homeostasis model assessment for insulin resistance and homeostasis model for ? cell function was calculated.Circulating CCN3,TNF-?,IL-6 and MCP-1 were determined by ELISA.OGTT was performed on 34 healthy individuals to explore the corelation between serum CCN3 and glucose.Results Serum CCN3 levels were significantly higher in nT2 DM patients compared to the healthy controls(6.71[4.88,8.56] vs.4.51[3.55,5.99] ng/ml,P < 0.01).Serum CCN3 positively correlated with BMI,WC,FAT%,TG,FPG,2h-PG,HbA1 c,FIns,HOMA-IR,hs-CRP and TNF-?,IL-6 and MCP-1,but negatively with HOMA-? in all individuals(P < 0.05).Multiple linear regression analysis indicated that BMI,HOMA-IR,TNF-? and MCP-1 were independently associated with CCN3.Multivariate logistic regression analysis demonstrated that CCN3 was correlated with nT2 DM.Finally,area under ROC curve of CCN3(gender and age adjusted)for predicting the presence of nT2 DM was 0.725(95% CI: 0.676-0.773).After an oral glucose challenge,there was no obvious change in the circulating levels of CCN3 as compared to 0 min(P > 0.05).Conclusion Newly diagnosed type 2 diabetes patients have elevated serum CCN3 level,which may be involved in obesity,insulin resistance and metabolic inflammation.
