ObjectivesTo compare the efficacy and benefit of percutaneous minimally invasive pedicle screw fixation and conventional open pedicle screw fixation for the treatment of type A thoracolumbar vertebral fractures,thereby guiding clinical practice and providing better clinical outcomes and treatment plan.MethodsFrom October 2016 to October 2018,patients with acute thoracolumbar junction vertebral fractures(T11,T12,L1,L2)of A1,A2 and A3 types who underwent simple short-segment transpedicular screw internal fixation in orthopaedics of Guangdong Hospital of Traditional Chinese Medicine were retrospectively analyzed.The patients in group A were all treated with minimally invasive percutaneous pedicle screw internal fixation(percutaneous pedicle screw internal fixation was used for injured vertebrae and adjacent upper and lower segments of injured vertebrae).The patients in group B were treated with traditional open pedicle screw internal fixation(common pedicle was used for injured vertebrae and adjacent upper and lower segments of injured vertebrae).The root screw was fixed internally.General information of two groups of patients was collected;pain degree was evaluated by VAS on the first day after operation and on the day of discharge;incision length,vertebral wedging(VW)and anterior vertebral height were compared between the two groups after operation and at the last follow-up;ODI index was used to evaluate the recovery of patients after operation.ResultsThere was no significant difference between the two groups in the loss of angle of vertebral body and the loss of anterior height of vertebral body(P>0.05);the VAS score of group A on the day of discharge was lower than that of group B(P<0.05);the ODI index of the last follow-up of the two groups was significantly different(P<0.05).ConclusionPercutaneous minimally invasive pedicle screw fixation for the treatment of type A thoracolumbar vertebral fractures is similar to traditional open pedicle screw fixation,but the operation time,intraoperative blood loss,postoperative There are obvious advantages in hospitalization time,etc.,which can be used as the main surgical method for the treatment of vertebral fractures of type A thoracolumbar junction under the premise of strict surgical indications.
