Objectives By studying the anatomical structure of normal nasolabial portion and the pathological mechanism of secondary deformity of cleft lip,the application and effect of herringbone costal cartilage in the repair of in secondary cleft lip rhinoplasty.Consequently,appropriate individualized therapeutic schemes are designed for the patients with secondary deformity of cleft lip.Methods From January 2017 to December 2018,a total of 40 cases of beauty seekers were performed in our hospital.Among them,including 20 cases with herringbone costal cartilage integral scaffold surgery and 20 cases with costal cartilage splicing stent surgery,the ages are 16 to 27 years old,with an average age of 19 years.There were no mental diseases,diabetes,hypertension and other chronic diseases in both groups.All of them were not pregnant or menstruating,and there were no infection lesions in the nose.Non-scar physique.Explain the difference between the two kinds of operation to all the patients before operation,ask them to choose the operation method independently,sign the informed consent letter.All the operations were successful.The patients were followed up immediately,1 month,2 months,3 months and 6 months after operation to record the operation time,the number of edema cases and the occurrence of postoperative deformations.The photographs of the patients were collected before and 6 month after the operation,and the photographs were compared before and after the operation.The nasal columella height,nasal basal width symmetry,nostril shape,nasal dorsum shape,alar symmetry and nasal tip shape were observed and compared in the two groups by qualitative method.It was proved that the operation improved the appearance of the affected side and affected the appearance of the contralateral side.At the same time,the effects of the two groups were compared.Results All the incisions healed in the first stage after operation.Only one patient had slight deformity of nasal contour after operation with autogenous rib cartilage splicing stent.After re-repair,the nasal contour was satisfactory and no deformity occurred again.Comparing the operation time of costal cartilage splicing stent with that of herringbone costal cartilage integral scaffold,the splicing stent group(3.0±0.5)h and the whole stent group(2.3±0.3)h were statistically analyzed(p< 0.05).The difference was statistically significant,indicating that the operation time of the whole stent was significantly shorter than that of the splicing stent.The number of cases of postoperative edema,p>0.05,the difference was not statistically significant,indicating edema both cases of postoperative number is basically the same;after deformation were P>0.05,and the difference was not statistically significant,the incidence is basically the same deformation of both methods after operation;There was no significant difference between the two groups in the symmetry of nasal base width,nasal foramen shape,nasal dorsum shape and alar symmetry(P>0.05).That is to say,the improvement effects of the two methods on the symmetry of nasal base width,nasal foramen shape and alar symmetry were similar,while the improvement scores on the height of nasal columella and nasal tip shape were similar(P<0.05).There was statistical significance between the two groups,that is,the operation method of human costal cartilage integral scaffold for repairing secondary nasal deformities of cleft lip was superior to costal cartilage splicing scaffold in the height of nasal columella and the shape of nasal tip.Conclusion 1.The repair of cleft lip and nose deformity with herringbone costal cartilage scaffold is a shorter operation time and less trauma.2.Human rib cartilage scaffold is more effective than splicing support in repairing the shape of nasal tip and the height of nasal columella.The effect is accurate and easy to be accepted by patients.3.The method of human costal cartilage integral scaffold for repairing nasal deformity secondary to cleft lip and costal cartilage splicing scaffold have stable effect,but the method of human costal cartilage integral scaffold improves the shortage of costal cartilage splicing scaffold operation method,which is simple and safe.It is worthrecommending this surgical method because of its good stability and strong supporting force.
