Objective To understand the status and factors with unplanned readmission among renal transplantation patients and to provide basis for targeted intervention.Methods In this study,a single-center retrospective case-control study was taken to select 424 patients who underwent renal transplantation from January 2012 to December 2016 in the jingling hospital.The incidence,causes and related influencing factors of unplanned readmission within one year after renal transplantation were analyzed by the self-designed questionnaire.Results1.Among 424 renal transplant patients,85(20.0%)had unplanned readmission within one year of discharge.In three months after discharge(64.7%),unplanned readmission of renal transplant patients occurred during the period of high incidence.Infection(50.6%)and acute rejection(15.3%)were the main causes of unplanned readmission.Logistic regression analysis showed that the number of complications(P=0.007),the estimation of glomerular filtration rate(P=0.006)and other operations during hospitalization(P=0.039)were independent risk factors for unplanned readmission in renal transplant recipients.2.43 among 424 renal transplantation patients were admitted to hospital because of infection,the readmission rate was 10.14%.The infection types included 24 cases of pulmonary infection(55.8%),11 cases of upper respiratory tract infection(25.6%)and8 cases of urinary tract infection(18.6%).34 patients with DCD infection(79.1%)and 9patients with linving related donor infection(20.9%).Logistic regression analysis showed that the postoperative hospital stay(P=0.032),hemoglobin(P=0.026)and the number of complications(P=0.022)were independent risk factors for unplanned readmission in infectious patients after renal transplantation.Conclusion The unplanned readmission within one year after operation was at a moderate level.Nursing staff should carry out continuous follow-up and dynamic evaluation,find out the risk factors and intervene effectively.Exspecially strengthen the management of patients with other operations were performed at the same time,more complications,lower e GFR,long postoperative hospital stay and low hemoglobin,So as to reduce the risk of readmission.The future research should establish an information-based,big data,multicenter cooperative platform for predicting the risk of readmission of infections in renal transplantation patients. |
