Clinical Research On Shunweijiangnifang Treating Refractory Gastroesophageal Reflux Disease

Objective: To observe the clinical curative effect of shunweijiangnifang treating Refractory Gastroesophageal Reflux Disease(RGERD)syndrome of disharmony in liver,spleen and stomach and mixture of cold-dampness-hot in TCM,so as to provide effective treatment and decoction for treating refractory gastroesophageal reflux disease,and to further promote its clinical application value.Method: In accordance with the principle of random control,72 patients conforming to this research were divided into treatment group of 36 patients and control group of 36 patients.The treatment group were administrated orally with shunweijiangnifang to be treated for 8 weeks,and the control group were administrated orally with esomeprazole magnesium enteric-coated tablets to be treated for 8 weeks.And RDQ,TCM syndrome classification and quantization table were filled out after and before treatment respectively,then observation and record were prepared.Additionally,the improvement change of gastroscopic esophagus mucous membrane inflammation before and after treatment was observed.Follow-up was performed after 4 weeks of treatment,and the recurrence rate was calculated..The results acquired were analyzed through SPSS20.0,so as to evaluate curative effect and make conclusions.Results: 1.RDQ score comparison showed that the treatment group had 4 patients who were healed,15 patients who had apparent effect,14 patients who had effect and 3 patients who had no effect,with total effective rate of 91.67%;while the control group had 2patients who were healed,6 patients who had apparent effect,18 patients who had effect and 10 patients who had no effect,with total effective rate of 72.22%.The comparison of the two groups showed that the treatment group was apparently better than the control group,the difference had statistical significance(P<0.05).2.The comparison of TCM syndrome curative effect showed that the aggregate score of the two groups of TCM syndrome were declined after treatment,and the treatment group was better than that of the control group,the difference had statistical significance(P<0.05).the the treatment group had 3 patients who were healed,12 patients who had apparent effect,16 patients who had effect and 5 patients who had no effect,with total effective rate of 86.11%;while the control group had 1 patients who were healed,6 patients who had apparent effect,10 patients who had effect and 19 patients who had no effect,with total effective rate of47.22%.The difference of the total effective rate for the two groups had statistical significance(P<0.05).The comparison between the two groups after treatment showed that the effect of the control group was better in the aspects of improvement of heartburn,belching,thirst,bitter taste,body fatigue,turgor,feces sticky,etc.than that of the control group,and the difference had statistical significance(P>0.05).While in the aspects of improvement of sour regurgitation,epigastric pain,substernal pain and poor appetite,the difference between the two groups had no statistical significance(P>0.05).3.Gastroscopic esophagus inflammation change showed that the total effective rate of the treatment group was 68.75%,the total effective rate of the control group was 92.86%,and the total effective rate of the two groups was not statistically significant(P>0.05).4.After 4 weeks of treatment,the two groups were followed up,the recurrence rate was 5.6% in the treatment group,27.8% in the control group,and the difference was statistically significant(P<0.05).Conclusion: The method of shunwenjiangnifang has a significantly clinical curative effect in RGERD syndrome of disharmony in liver,spleen and stomach,and mixture of cold-dampness-hot in TCM,it can improve gastroscopic esophagus inflammation,reduce the recurrence rate and improve living quality of the patients,so as to provide effective treatment and decoction for treating refractory gastroesophageal reflux disease.
