Objective:(1)in this study,endoscopic sinus puncture of the blood collatuses in the nasal cavity was carried out under direct vision,and the traditional therapy of traditional Chinese medicine for perennial allergic rhinitis was innovated,and the clinical efficacy of nasal sinus puncture of the blood collatuses under nasal endoscope in the treatment of perennial allergic rhinitis was evaluated.(2)to explore the mechanism of this method in the treatment of perennial allergic rhinitis by observing the effect of nasal hilus and neiyinghong acupoint on eosinophils,serum total IgE and serum IL-4 in patients with perennial allergic rhinitis under nasal endoscopy.Methods: Sixty outpatients in the department of otolaryngology of the first affiliated hospital of anhui university of traditional Chinese medicine were selected.The patients were randomly divided into two groups,namely,30 patients in the treatment group of blood collateral puncture at the nasal mound and neiyingxiang point under nasal endoscope,and 30 patients in the control group of fluticasone nasal spray.In the treatment group,the nasal colliculus and neiyingxiang point were used to puncture blood collaterals under nasal endoscope.The control group was given fluticasone propionate nasal spray.AR symptoms and signs were scored before treatment,1 week after treatment,2,3,4 weeks after treatment,and 4 weeks after follow-up.Venous blood and nasal secretions were collected 24 hours before treatment and 4 weeks after treatment.The content of eosinophils in nasal secretion was determined by immunohistochemistry.Results:Comparison of symptoms and signs scores and clinical efficacy:after treatment,it is found that the scores of the two groups before and after treatment are significantly different(P<0.01),and the advantage of the treatment group and the control group isobviously higher than that before treatment.Meanwhile,after the treatment,the two groups showed no significant difference(that is,P>0.05).Compared the efficiency of the treatment group with that of the control group,it found that there was no significant difference(that is,p>0.05).Elisa method:compared with that of control group,the serum IgE and il-4 levels of venous blood of the treatment group showed no significant difference(that is,p>0.05).The difference of serum IgE and il-4 level between the two groups before and after treatment is significant(P<0.01).Compared with that of control group,the IgE and il-4 levels of venous blood in the treatment group showed no significant difference(that is,p>0.05).HE staining method:compared with that of the control group,the level of EOS of nasal secretions in the treatment group showed no obvious difference(P=0.6844,P> 0.05).The EOS level of nasal secretions of the two groups was significantly different before and after treatment(P<0.01).Compared with that of the control group,the level of nasal discharge EOS in the treatment group showed no obvious difference(after treatment).P=0.6408,P>0.05.Conclusion:The clinical symptoms of perennial allergic rhinitis were significantly improved under nasal endoscopy,and the curative effect was definite.In addition,the secretion of serum IgE and IL-4 was inhibited,and the infiltration of nasal secretion EOS was reduced.The mechanism of this method in the treatment of perennial allergic rhinitis was further discussed.
