Objective:Pulmonary nodule(PN)is often a manifestation of early lung cancer.Early differential diagnosis of PN can effectively detect early lung cancer and improve the survival time of the lung cancer patients.By retrospectiving analysis of solitary pulmonary nodule(SPN)patients admitted to the Huai He hospital of The He Nan University in the past six years,the clinical symptoms were analyzed to explore the value of fibrinogen and platelet in the differential diagnosis of SPN,such as age,gender,fibrinogen,platelet,pathological results and so on.Methods:The 80 patients with SPN from the Huai He hospital of The He Nan University in recent six years were retrospectively analyzed.They had complete medical records,platelet and fibrinogen detection values,chest CT and pathological diagnosis.The data was divided into the lung cancer group and benign lesions by pathological diagnosis.The patients'age,sex,smoking history,the parts of the nodule,platelet count,fibrinogen concentration are observation indexes,such as measurement data using(?x+s),comparison between groups using independent sample t test,the counting data were examined by?~2test,The P value is lower0.05,that he difference is statistically significant.Results:1.The 80 patients with SPN were included,whose age fluctuated from 31-84 years old,among which50-60 years old was the age with high incidence of pulmonary nodules.2.Among the 80 cases of SPN included,cough accounted for 38.75%,expectoration 31.25%,and physical examination found 25.00%.SPN lacked characteristic clinical manifestations in early stage.3.A total of 80 SPN patients were included and the pathological results were statistically analyzed.Among them,27 were adenocarcinoma,accounting for 33.75%,followed by squamous cell carcinoma25.00%,which was consistent with the clinical characteristics of lung cancer.4.The age,gender,smoking history and nodule site of the 80 patients were statistically analyzed,and the P values of age,gender,smoking history and nodule site were all greater than 0.05,with no statistically significant difference between the two groups.5.The SPN imaging Which include vascular cluster sign,leaf-splitting sign,pleural depression sign can help diagnose the benign and malignant SPN.The t values is lower than 0.05,the difference was statistically significant.6.The platelet count and fibrinogen concentration of the 80 patients with SPN were statistically analyzed in the benign lesion group and lung cancer group.The t values is 1.655,the P value was greater than 0.05,the difference was not statistically significant.7.The plasma platelet count of SPN in the patients with adenocarcinoma and squamous cell carcinoma was analyzed,and the results of statistical analysis showed that the t values is-0.676,the P value wasl greater than 0.05,which means no statistical significance.8.The plasma FIB concentration of adenocarcinoma and squamous cell carcinoma in SPN was analyzed,and the results of statistical analysis showed that the t values is-3.260,the P value was lower than0.05,which means statistically significant differences.Conclusion:The results of this study suggest that the age,sex,smoking history and nodule location PLT countand FIB concentration of the SPN patients were not risk factors for predicting benign and malignant nodules.SPN imaging Which include vascular cluster sign,leaf-splitting sign,pleural depression sign can help diagnose the benign and malignant SPN.The FIB concentration in SPN squamous cell carcinoma patients was higher than that in adenocarcinoma patients.The FIB concentration could assist clinical classification of lung squamous cell carcinoma and adenocarcinoma.Due to the small sample size,further confirmation is needed.
