ObjectiveThis study analyzed the curative effects of primary total hip arthroplasty with RM Pressfit Cup retrospectively,providing a reference for the selection of acetabular prosthesis for total hip arthroplasty.Materials and methodsAll clinical datas from October 2014 to May 2018 were obtained from Huaihe hospital.Primary total hip arthroplasties were performed in our department with RM Pressfit Cup acetabular prostheses for all 96 patients.Joint replacement reason: femoral neck fracture 26 patients(26 hips)?Necrosis of femoral head 58 patients(66 hips);Developmental hip dysplasia 4 patients(4 hips).The acetabular sides were fixed with titanium metal coated RM PressfitCup biologically.The biologic fixation,periprosthetic osteolysis and integration of the acetabular prosthesis were evaluated according to the postoperative imaging data ?Harris score?VAS score and SF-36 score of the hip joint,and evaluate hip function and patient satisfaction.Preoperative and postoperative imaging datas were used to evaluate the position,loosening and osteolysis of the prosthesis.Results92 cases were completely followed up for an average follow-up time of(27.0 ± 4.5)months,the Harris score of hip joint increased from(42.9±9.3)before surgery to(80.7± 5.3)at 6 months after surgery and(86.1±6.2)at the last follow-up.At the last follow-up,the Harris score was improved compared with that before and 6 months after surgery significantly,and the differences were statistically significant(t =281.96,P < 0.05;t =30.10,P < 0.05).VAS score decreased from(4.4±1.7)before surgery to(1.1±0.8)at 6 months after surgery,and was(0.7±0.6)at the last follow-up,with statistically significant differences(t =474.82,P < 0.05;t=43.41,P < 0.05).The SF-36 score was(48.7±3.6)before the operation,and(86.3±7.6)6 months after the operation.The score at the last follow-up was(88.1±8.3).The SF-36 score was significantly improved 6 months after operation compared with that before operation(t =203.21,P < 0.05).One patient was followed up with dislocation and no abnormal hip pain was found.Postoperative follow-up and physical examination showed that the range of motion and function of the hip were significantly improved compared with those before surgery,and the range of motion of flexion and extension of the affected hip was(42.2± 12.3)°,the range of adduction and abduction was(10.1 + 1.9)°,6 months after surgery and at the last follow-up,the range of flexion and extension of the hip joint were(103.9± 9.1)°?(114.6±13.5)°.The range of adduction and abduction were(31.2 + 3.3)°?(39.8 + 4.5)°.There were statistically significant differences between the 6 months after operation and the last follow-up and that before operation(t=313.24,P < 0.05).The patients basically satisfied the daily activities and squat movements.No revision was found at the last follow-up.Pelvic X-ray examination after the surgery showed no significant changes in the rotation center of the acetabular prosthesis or the loosening or displacement of the prosthesis,and no significant bone resorption or dissolution around the prosthesis.ConclusionIn the initial total hip arthroplasty,RM elastic acetabular prosthesis has a reliable initial stability,which can improve the function of the affected hip effectively.Peripheral sclerotic bone is significantly improved compared with that before surgery,and the bone integration effect is good,with satisfactory recent clinical and imaging results.
