Objective:In this study,we examined the expression of interleukin-5 and CysLTs in nasal cavity and sinus mucosa of patients with ECRS(eosinophilic chronic rhinosinusitis,ECRS)and NECRS(noneosinophilic chronic rhinosinusitis,NECRS),and explored the possible role of IL-5 and CysLTs in the pathogenesis of ECRS and the possible treatment of ECRS.Methods:122 patients who underwent FESS(functional endonasal sinus surgery)in our hospital from September 2017 to September 2018 were selected.Before operation,serious cardiovascular and cerebrovascular diseases,bronchial asthma,allergic diseases or other diseases that may affect the experiment were excluded through detailed inquiry of the medical history.All patients were routinely examined for EOS(eosinophilie)in peripheral blood,CT and endoscopy of sinuses and other necessary preoperative examinations.Each patient was scored with VAS scale and CTLund-Mackay score of nasal sinuses.According to the EOS count after HE staining,ECRS group was included if the field of vision per high power mirror(x400)> 5,and NECRS group if the number of EOS per high power mirror(x 400)< 5.Each group can be divided into ECRSwNP(eosinophilic chronic rhinosinusitis with nasal polyps),ECRSsNP(eosinophilic chronic rhinosinusitis without nasal polyps),NECRSwNP(noneosinophilic chronic rhinosinusitis with Al polyps),NECRSWP(noneosinophilic chronic rhinosinusitis without nasal polyps).The expression of IL-5and CysLTs in each group was counted by immunohistochemical method,and their correlation with EOS infiltration and disease severity was analyzed.Results:1.EOS counts in peripheral blood and tissue of 122 patients were 0.29(+0.22)and 12.72(+16.25)respectively,and there was a positive correlation between them(r=0.860,P < 0.001).2.VAS scores of all patients were 4.36(+2.16)and CTLund-Mackay scores of nasal sinuses were 8.14(+4.79),and the degree of tissue EOS infiltration was positively correlated with them(r=0.887,0.885,P < 0.001).3.The expression of IL-5 in ECRS group was significantly higher than that in NECRS group(0.72 +0.12),t = 19.901,P < 0.001;4.The expression of CysLTs in ECRS group was significantly higher than that in NECRS group(0.35 +0.03),t = 26.073,P< 0.001;5.The expression of IL-5 and CysLTs in ECRSWNP group(0.76 +0.12 and0.35 +0.03)were higher than that in NPS group(0.06 +0.06 and 0.03,respectively).0.33 + 0.02),t = 4.416,P < 0.001;t = 2.059,P < 0.05.6.The expressions of IL-5 and CysLTs in NECRSwNP group(0.41 +0.03 and 0.25 +0.02,respectively)were higher than those in ECRSsNP group(0.24 +0.04 and 0.15 +0.01,respectively),t = 14.899,P< 0.001,t = 27.945,P < 0.001.Conclusion:1.Compared with NECRS,the expression of IL-5 and CysLTs in nasal cavity and sinus mucosa of ECRS patients was significantly increased,and positively correlated with the infiltration of EOS.4.The expression of IL-5 and CysLTs in nasal cavity and sinus mucosa of ECRS patients was positively correlated with the infiltration of EOS.4.The expression of IL-5 and CysLTs was positively correlated with the infiltration of EOS.It may be related to the mechanism of nasal polyp formation.
