Optimization Of Key Techniques In Laparoscopic Hepatectomy For Intrahepatic Cholelithiasis

Objective:To refine and optimize the key techniques of laparoscopic hepatectomy in the treatment of intrahepatic cholelithiasis.Methods:Laparoscopic hepatectomy was performed tHepatobiliary o treat 30 patients with intrahepatic cholelithiasis in the Department of surgery,Laparoscopic surgery was performed by a five-hole method.The operation were performed according to the following technical process:blocking the blood flow of the diseased liver lobe(segment)into the liver,Liver and duodenal ligaments were dissected and dissected.If necessary,To remove the stones from the ruptured end of the left hepatic duct,and than a T tube or primary suture was applied.The data of preoperative general information,imaging examination,The surgical methods of patients were collected and analyzed.The key technologies,including imaging examination(three-dimensional visualization of liver),usage of intrahepatic space and so on,were excavated in depth according to the data.Results:Laparoscopic anatomic left hemihepatectomy was performed in 14 cases and left lateral lobectomy in 16 cases.Median operating time is 256(100-480)min,in which the left hemihepatectomy was 270(180-480 min),and left lateral lobectomy 240(100-480)min.Four patients were given the appropriate blood.Of the 23 cases,2 cases were distal to the left hepatic duct and 21 cases were through common bile duct.Primary suture of common bile duct were performed in 2 cases and T-tube drainag in 19 casese.Neither serious complications nor death was found perioperative period.29 cases were followed up for 15 months(1-27 months).1 case was removed with choledochoscope,the other 1 case refused further treatment,the complete stone clearance rate was 96.6%(28/29).Conclusion:1.Laparoscopic hepatectomy is safe and feasible for the treatment of intrahepatic cholelithiasis.precise surgical planning,Seiko Surgery and Excellent Postoperative Managementthe.It ean achieve ideal therapeutie effect,surgical safety and invasive intervention.2.The key techniques of laparoscopic hepatectomy in the treatment of intrahepatic cholelithiasis include accurate preoperative evaluation,precise liver section using intrahepatic space,removal of other bile duct stones and so on.3.The reasonable application of preoperative three-dimensional visualization,meticulous dissection of hilar plate,precise positioning of diseased segment and usage of intrahepatic space in laparoscopic hepatectomy contribute to the decrease of operation time,Intrahepatic hemorrhage,...
