Objective: To compare the efficacy of the Wiltse approach with the traditional posterior approach in the treatment of thoracolumbar fractures without spinal cord injury.Methods: From January 2014 to December 2017,patients with spinal surgery in the Qijishan Hospital of Wannan Medical College were admitted to the Department of Thoracic and Thoracic Fracture and received surgery.A total of 24 patients were enrolled in the Wiltse approach group,12 males and 12 females.12 patients,the age of the patient was 20-59 years old,the average age was(45.29±9.67)years old;the traditional posterior median group was 26 patients,14 males and 12 females,the patients were 31-66 years old,and the average age was(49.15±7.77)years old.The operation status,preoperative and postoperative imaging parameters,and Visual Analogue Score VAS were compared between the two groups.Results: 1)In the Wiltse approach group,the operation time,the blood loss during operation and the postoperative drainage was compared with the traditional posterior approach.The difference was statistically significant(P<0.05).2)The height of the injured vertebrae and the sagittal Cobb angle were significantly improved in the two groups.The patients in the Wiltse approach group were treated with the height of the injured anterior border and the traditional posterior median group before,one week after operation,last follow-up.The difference was not statistically significant(P>0.05).3)The postoperative VAS scores of the two groups were significantly improved.There was no significant difference in the preoperative VAS score between the Wiltse approach group and the traditional posterior median group(P>0.05).The Wiltse approach group was postoperatively.The VAS score of one week was lower than that of the traditional posterior approach group(P<0.05).The VAS score of the Wiltse approach group was lower than that of the traditional posterior approach group.The difference was statistically significant(P<0.05).).Conclusion: The Wiltse approach and the traditional posterior approach can achieve good clinical results in the treatment of thoracolumbar fractures without spinal cord injury,but the Wiltse approach has short operation time,less blood loss during surgery,and less postoperative drainage,in the postoperative VAS score,it is superior to the traditional posterior approach group,which can reduce the postoperative low back pain.It is an easy to carry out and worthy of promotion.
