Objective:To Study Clinical significance of eosinophil in stratified treatment of patients with acute exacerbation of chronic obstructive pulmonary disease Methods: 332 cases of acute exacerbation of chronic obstructive pulmonary disease(Acute Exacerbation of Chronic Obstructive Pulmonary Disease,AECOPD)were collected from October 2016 to March 2018 in the respiratory department of Yi Ji Shan Hospital.The patients were divided into eosinophil positive group and negative group according to Eosinophilic granulocyte percentage 2% for cut-off value.The usage and duration of glucocorticoid(glucocorticoid,GCS)during hospitalization were compared between the two groups(Standardization of hormone use based on hospitalization days),The percentages of neutrophils,leukocytes,years of smoking,CRP,ESR,pulmonary function test(FEV1%pre,FEV1/FVC%)and arterial blood gas analysis(PaO2,PaCO2)were counted in the two groups.CAT score,arterial blood gas,sex,age,etc.The data results were analyzed by SPSS 18.0 software,the measurement data were expressed by x± s,the independent sample T test was used,the counting data were analyzed by ? 2 test,and the correlation of counting data was analyzed by Pearson correlation analysis.The difference was statistically significant.(p < 0.05)Results:A total of 332 patients with AECOPD were selected and 280 patients were selected after exclusion criteria.They were divided into eosinophil positive group(?2%,69 cases)and eosinophil negative group(< 2%,221 cases)according to the cut-offvalue of 2% eosinophil,There was no significant difference in smoking history,age,sex,severity,number of exacerbations and common complications between the two groups(Table 2?Table 4?Table 5).There was significant difference between the two groups in terms of hospitalization time and convenience of hormone usage.The hospitalization time and hormone usage in positive group were less than those in negative group,and there was statistical difference between the two groups.(Table 5?Table 6?Table7)Conclusion: This study showed that patients with eosinophil count ? 2% were hospitalized,and the use of hormone was significantly lower than that of patients with eosinophil count < 2%.According to the level of eosinophil count,we can guide and standardize the dosage of hormone in patients with AECOPD.;The frequency of eosinophil aggravation in the positive group was more frequent than that in the negative group,and the quality of life was worse than that in the negative group.The secondary eosinophil count level could be used as one of the indexes to predict the aggravation of COPD.
