Objective:The purpose of this study was to investigate the relationship between the expression of serum mi RNA-221,P53 and chemosensitivity in patients with gastric cancer,and to explore whether there were differences between the expression of serum mi RNA-221,P53 in patients with gastric cancer and healthy controls,and to analyze the correlation between serum mi RNA-221,P53 and clinicopathological parameters,immunohistochemical indexes and serological tumor marker CEA of gastric cancer.Methods:60 patients with neoadjuvant chemotherapeutic gastric cancer were selected as experimental group1,40 patients with surgical treatment of gastric cancer as experimental group 2,and 40 healthy persons as control group.The expression of serum mi RNA-221 and P53 in peripheral blood of all experimental group and control group was detected by RT-PCR.,and at the first,to compare the difference of the expression of serum mi RNA-221,P53 in gastric cancer group and normal group.Subsequently,the chemotherapy efficacy of 60 patients with neoadjuvant chemotherapy for gastric cancer in the experimental group was evaluated.After the evaluation,the efficacy of chemotherapy and the ineffective chemotherapy were analyzed according to the effect of chemotherapy,and to compare the levels of serum mi RNA-221,P53 expression were compared between the two groups.Then,according to the effect of chemotherapy,it was subdivided into four groups: complete remission group,partial remission group,disease stabilization group and disease progression group.The expression of serum mi RNA-221 and serum P53 was compared between the groups,and serum mi RNA-221 and serum P53 were evaluated for chemotherapy sensitivity to gastric cancer.Finally,the clinical and pathological data of 40 patients in experimental group 2 were collected,including age of onset,histopathological and immunohistochemical features,and the correlation between the expression of serum mi RNA-221,P53 and each index was analyzed.Results:1.The expression of mi RNA-221 in three gastric cancer cell lines SGC7901,AGS,BGC823 was significantly higher than that in normal gastric mucosal cells GES-1 cells.2.Quantitative detection of serum mi RNA-221 and P53 in all patients in experimental and control groups,the results are as follows:2.1.The expression of serum mi RNA-221 in patients with gastric cancer was significantly higher than that in healthy controls,and the difference was statistically significant(p?< 0.05).2.2.To analysis of serum mi RNA-221 and chemosensitivity of gastric cancer,the results are as follows:2.2.1 The expression of serum mi RNA-221 in patients with ineffective chemotherapy was significantly higher than that in patients with ineffective chemotherapy,and the difference was statistically significant(t = 4.163,? < 0.001).2.2.2 The expression of serum mi RNA-221 in the progressive group(PD group)was significantly higher than that in the stable group(SD group)and the partial remission group(PR group).At the same time,the expression of serum mi RNA-221 in stable group(SD group)was significantly higher than that in partial remission group(PR group),and the differences was statistically significant(? < 0.05).;2.3 The expression of p53 in serum of patients with gastric cancer was higher than that of healthy controls(? < 0.05),and the difference was significant(? < 0.05);2.4 To analysis of serum P53 and chemosensitivity of gastric cancer,the results are as follows:2.4.1 There was no significant difference in the expression of serum P53 between the two groups(t = 0.447,?= 0.657);2.4.2 There was no significant difference in the expression of serum p53 between disease stable group and the partial remission group and the disease progression group(F = 1.401,?= 0.255).2.5.By analyzing the predictive value of serum mi RNA-221,P53 alone and combined to neoadjuvant chemotherapy sensitivity of gastric cancer,it was concluded that the specificity of serum mi RNA-221 in predicting neoadjuvant chemosensitivity of gastric cancer was higher;3.The expression level of serum mi RNA-221 was significantly higher in the larger diameter group,the poorly differentiated tumor tissue group,the lymph node metastasis group and the tumor tissue E-cadherin expression group than the tumor diameter group(t=2.593,?=0.015).,tumor tissue differentiation group(t=4.01,?=0.001),no lymph node metastasis group(t=2.714,?=0.011)and E-cadherin expression normal group(t=3.504,?=0.001),but at different ages There were no significant differences in the expression of groups,different tumor sites,and C-erb B-2 and Ki67 expression groups.4.Serum P53 expression was higher in the lymph node metastasis group than in the no lymph node metastasis group,and the difference was statistically significant(t=2.070,?=0.047),but in different age groups,different tumor sites,different tumor tissue differentiation groups and Ki67.There was no significant difference in the expression of different groups.Conclusion:1.The expression of serum mi RNA-221?P53 in gastric cancer group was significantly higher than that in healthy control group.2.To detect the expression of serum mi RNA-221,P53 is helpful to evaluate the prognosis of patients with gastric cancer after surgical treatment.3.The expression of serum mi RNA-221 was correlated with the chemosensitivity of gastric cancer patients with neoadjuvant chemotherapy;There may be no significant correlation between serum p53 level and chemotherapy sensitivity in patients with gastric cancer treated with neoadjuvant chemotherapy.There was no significant difference between the detection of serum mi RNA-221 by combined detection and the sensitivity of chemotherapy alone.
