Objective: The incidence of Alzheimer’s disease is increasing year by year,which has become an important disease affecting the health of the elderly.With people’s continuous attention to the pathogenesis,pathology,anatomy,clinical differentiation and drug treatment of AD,the criteria for the identification of AD are gradually improved,from a single clinical symptom to the integration of clinical,molecular genetics,brain neuroimaging and other factors.However,effective prevention and treatment of AD have not been found so far,so we need to find new effective prevention and treatment of AD,which has become a research hotspot and research direction,and providing new ideas for early intervention of AD.Mild cognitive impairment,a cognitive impairment between normal aging and dementia,is a clinical syndrome that can develop into AD.Studies at home and abroad have found that the autobiographical memory of AD patients is characterized by overgeneralization,and the autobiographical memory is seriously damaged.However,less attention has been paid to the autobiographical memory of MCI patients.This topic will study the clinical characteristics of the autobiographical memory of MCI patients,so as to carry out early identification and intervention for MCI patients and delay the process of MCI developing into AD.Methods: A total of 50 MCI patients and 57 healthy elderly patients who met the inclusion criteria were selected from the Alzheimer’s disease diagnosis and treatment center of Wuxi community and Wuxi Mental Health Center as the control group,with ages ranging from 60 to 75 years old and education levels ranging from primary school to university.Subjects in MCI group and healthy elderly group were tested with Mini-mental State Examination,the geriatric depression scale-11,Halstead Reitan Neuropsychological Battery Revised in China and Autobiographical Memory Test.In this study,SPSS22.0 software was used to conduct independent sample t-test,correlation analysis and multiple linear regression analysis on the test results of the two groups of subjects.Results: The autobiographical memory scores of MCI group and healthy elderly group were tested by independent sample t test.The results showed that there was significant difference in the scores between the two groups(p < 0.05).The results showed that the extraction accuracy of the cued words in the specific memory and autobiographical memory of the healthy elderly group was significantly better than that in the MCI group.The difference of categorical memory between the healthy elderly group and the MCI group was statistically significant(p<0.05),while the expression of expandable memory was not statistically significant between the two groups(p>0.05).In the autobiographical memory test,the forgetting degree of the MCI group was higher than that of the healthy elderly group,indicating that the autobiographical memory of the MCI group was difficult to extract,that is,the autobiographical memory was overgeneralized.There was significant difference in the total score of the neuropsychological test between the healthy elderly group and the MCI group(p < 0.05).The total score of the core neuropsychological test and the score of each sub-test in the healthy elderly group were better than those in the MCI group except for the results of the mathematical and physical operation test(p < 0.05).The total score of autobiographical memory and specific memory was correlated with the total score of core neuropsychological test and the score of each test,but the correlation was low,indicating that the decline of autobiographical memory may affect the score of core neuropsychological test.Multivariate linear regression analysis shows that autobiographical memory has a linear regression relationship with number span,graph matching and line test in core neuropsychological test.Conclusion: The autobiographical memory of patients with MCI is difficult to extract,which is mainly manifested in the difficulty of extracting some clue words and forgetting,that is,the autobiographical memory of MCI patients is characterized by overgeneralization.The decline of autobiographical memory in patients with MCI may affect the performance of executive function and cognitive function,suggesting that overgeneral autobiographical memory can be used in the neuropsychological assessment of normal elderly,in addition,providing a reference for the differentiation criteria of neuropsychology with MCI patients in the future. |
