Objective:The purpose of this study is to observe the effect evaluation and prognosis analysis of CT-guided 125I seed implantation combined with chemotherapy in the treatment of small cell lung cancer,so as to provide reference for clinicians in the treatment of small cell lung cancer.Methods:This study retrospectively analyzed 45 cases of small cell lung cancer treated by radioactive 125I seed implantation combined with chemotherapy in our respiratory department from March 2013 to March 2017.Detailed records of the selected patients?personal information,tumors,chest CT findings,etc.?were followed up regularly to observe the clinical efficacy,changes in tumor markers and survival time.The follow-up deadline was December 31,2018.Results:1.Therapeutic evaluation:45 patients according to RECIST 1.1 solid tumor standard,1 month after operation:3 cases of complete remission?CR?,32 cases of partial remission?PR?,10 cases of stable condition?SD?,0 cases of disease progression?PD?,the total effective rate?CR+PR?was 77.8%;while the 3 months after operation:3 cases of CR,29 cases of PR,8 cases of SD,5 cases of PD,the total effective rate?CR+PR?was 71.1%;while the 6 months after operation:2 cases of CR,27 cases of PR,4 cases of SD,The total effective rate?CR+PR?was 64.4%.2.Neuron-specific enolase?NSE?decreased significantly after operation compared with that before operation?53.35±59.87?before operation,18.52±21.48)after operation,t=5.807,P<0.05).3.Survival period:half-year survival rate was 82.2%?87.5%in the limited period and 79.3%in the extensive period?;one-year survival rate was 66.7%?75.0%in the limited period and 62.1%in the extensive period?;average progression-free period?mPFS?was 11.1 months;median survival time?MST?was 13 months,including 17 months in the limited period and 12 months in the extensive period.4.Cox proportional hazard model showed that age?HR=1.250,95%CI:1.109-2.761,P=0.003?,disease stage?HR=6.285,95%CI:3.602-9.783,P=0.002?,pulmonary complications?HR=2.774,95%CI:1.051-5.296,P=0.019?were the main factors affecting prognosis.Conclusions:1.CT-guided radioactive125I seed implantation combined with chemotherapy has a good short-term therapeutic effect on small cell lung cancer patients.It has a high local control rate and is an effective and advisable treatment.2..Age,stage,pulmonary complications?pneumonia,atelectasis,pleural effusion,etc.?are important factors affecting survival and prognosis of patients with small cell lung cancer treated by125I seed implantation combined with chemotherapy.
