Objective:The purpose of this study was to compare and analyze the relationship between the rapid increase of body weight after operation of Craniopharyngioma and the content of serum nesfatin-1 through the determination of serum nesfatin-1 content in normal healthy population and craniopharyngioma before operation,and to explore the correlation between nesfatin-1 and hypothalamic obesity.Provide new ideas for the treatment of hypothalamic obesity Methods:Started screening in September 2017 for neurosurgery patients in the first affiliated hospital of Nanchang University,the preoperative MRI examination and biochemical examination were suggested as Craniopharyngioma and agreed to surgical treatment after understanding the condition.For all the above patients,the preoperative venous blood samples were collected after the informed consent operation,and the postoperative pathological results suggested that the patients with Craniopharyngioma were also collected from 3,14,30,90 and 180 days after operation,respectively,to collect their clinical follow-up data and blood samples;According to the characteristics of the age group of craniopharyngioma and the sex ratio of the patients in the data collected,the basic information and blood samples of 10 volunteers in outpatient medical examination were collected.Through the analysis of clinical data,20 cases of postoperative hypothalamic damage and significant changes in body mass index after operation were screened.The monthly weight gain rate and body mass index of the normal control group and the experimental group within 6 months were calculated according to the formula respectively,and the corresponding graphs were recorded and plotted,and the nesfatin-1 content was determined by enzyme-linked immunosorbent test,and all the data were treated with SPSS21.0,Pearson correlation analysis and multivariate linear regression analysis were used to understand the relationship between Nesfatin-1 and hypothalamic obesity.Results:1.Compared with the normal health group,the level of Nesfatin-1 before operation of Craniopharyngioma was significantly lower,and compared with the preoperative comparison of Craniopharyngioma,the level of Nesfatin-1 was significantly increased 3 days after operation,the level of Nesfatin-1 decreased significantly after 2 weeks,and the level of Nesfatin-1 decreased significantly after 1 months after operation.The level of Nesfatin-1 decreased obviously 3 months after operation,and the level of Nesfatin-1 decreased obviously 6 months after operation.2.Compared with the normal health group,there was no significant change in BMI before operation of Craniopharyngioma(p>0.05);Compared with the preoperative comparison of Craniopharyngioma,the BMI did not change significantly after 3 days of operation(p>0.05),the BMI did not changed significantly after 2 weeks(p>0.05),the BMI increased obviously after 1 months(p<0.05),the BMI increased significantly after 3 months(p<0.05),and the BMI increased significantly after 6 months after operation(p<0.05).3.The serum Nesfatin-1 level of 2 weeks,1 months,3 months and 6 months after operation was positively correlated with BMI(p<0.05),and there was no significant correlation with sex and age(p>0.05).Compared with preoperative comparison,2 weeks,1 months and 3 months of postoperative ?Nesfatin-1 were negatively correlated with ?BMI(p<0.05),?Nesfatin-1 was positively correlated with ?BMI 6 months after operation(p<0.05).4.Using serum Nesfatin-1 as dependent variable,gender,age and BMI as independent variables for multivariate stepwise regression analysis,it was found that the above indexes were not correlated with serum Nesfatin-1(p>0.05),and the ?Nesfatin-1 was further used as the dependent variable,stepwise regression analysis with ?BMI as an independent variable showed that ?BMI was an independent influencing factor of serum Nesfatin-1(?=0.398,p<0.05).Conclusion:1.The serum Nesfatin-1 value of patients with Craniopharyngioma was significantly lower than that in the healthy control group and the BMI was higher,which may be related to the invasion or compression of hypothalamus by Craniopharyngioma.2.There was a negative correlation between Nesfatin-1 and BMI after craniotomy,suggesting that Nesfatin-1 may have been involved in the development of hypothalamic obesity after craniopharyngioma.3.The level of Nesfatin-1 increased significantly 6 months after surgery,suggesting that hypothalamic obesity may be compensated.
