Objective:To Improve the injected water in the balloon of foley catheter to make swelled initial side blocked the aorta descendens combined with moderate hypothermia circulatory arrest;as well as making a research of organ protection in operative treatment for actue Stanford A type aortic dissection.Methods:To make a retrospective study including data of total amount of 68 patients underwent type A aortic dissection whose had recived Sun's operation in the Second Affiliated Hospital of Nanchang University from October 2015 to January 2019.Dividing data into two groups according to the method of aortic arch blocking and the temperature of circulatory arrest.One group is control group(n=34)that adopt strategy of classic Sun's operation with circulatory arrested 30 min in 25?(anus temperature).Another group is observation group(n=34)that injected water in the balloon of foley catheter to make swelled initial side blocked the aorta descendens,and achieved the aim of temporary(3-5min)circulatory arrest in 25?(anus temperature).Collecting data of patient's general condition before recived operation as well in and after operation,to analyze the clinical curative effect of two groups.Results:Three patients died after had recived classic Sun's operation in control group,beside three patients died after recived evolutionary Sun's operation in observation group.There was no significant difference in the time of blocking aortic arch and cardiopulmonary bypass between the two groups.At the meanwhile observ-ation group have shorter time of heating,cooling,cardiopulmonary bypass,postoper-ative awake,mechanical ventilation,stayed in ICU,and stayed in hospital than control group,beside have better organ protection like brain,liver and kidney than control group.Conclusion:Choosing the evolutionary strategy of blocking aortic arch can reduce the time ofcirculatory arrest,which can bring the advantage of organ protection,and get proper length of trunk stent to make operation variable.In conclusion,this strategy is worth to promote in clinic.
