| Objectives:The clinical effect of CT-guided microwave ablation in the treatment of different tumor diameters compared with surgical resection of primary hepatocellular carcinoma.To investigate the efficacy and superiority of microwave ablation for primary hepatocellular carcinoma patients with different tumor diameters.Methods:A retrospective analysis was made of 120 patients with primary hepatocellular carcinoma diagnosed pathologically in the Second Affiliated Hospital of Nanchang University from January 2012 to December 2015.The selected cases were 60 patients(30 patients with tumor diameter less than 3 cm,30 patients with tumor diameter 3-5cm)and 60 patients(30 patients with tumor diameter less than 3 cm,30 patients with tumor diameter 3-5 cm)who underwent surgical treatment.The clinical data of the two groups were collected to observe the curative effect and recovery of the two groups.The patients in both groups were followed up for 3years.The recurrence rate,tumor-free survival rate and total survival rate of the patients in 1,2 and 3 years after operation were counted.Results:Compared with the operation group,the microwave ablation group had shorter operation time,less bleeding,shorter hospitalization time,less hospitalization expenses and fewer perioperative complications.Patients with tumor diameter less than 3 cm: Compared with the operation group,the liver function recovered quickly after microwave ablation.The recurrence rates of microwave ablation group and resection group were 10/30 and 9/30,respectively.There was no significant difference between the two groups(P > 0.05).The 1-year,2-year and 3-year disease-free survival rates in the microwave ablation group were 22(73.3%),15(50.0%)and 13(43.3%)respectively,while those in the operation group were 23(76.7%),17(56.7%)and 13(43.3%).There was no significant difference between the two groups during the same period(P > 0.05).There was no significantdifference in the 3-year overall survival rate between the microwave ablation group and the operation group(P > 0.05).Patients with tumors 3-5 cm in diameter: The recurrence rates of microwave ablation group and operation group were 21/30 and 10/30 respectively.There was significant difference between the two groups(P < O.05).The 1-year,2-year and3-year disease-free survival rates in the microwave ablation group were 19(63.3%),14(46.7%)and 4(13.3%)respectively,while those in the operation group were 20(70.0%),15(53.3%)and 12(40.0%).There was no significant difference in the1-year and 2-year disease-free survival rates(P > 0.05),but there was a significant difference in the 3-year disease-free survival rates between the operation group and the microwave ablation group(P < 0.05).There was no significant difference in the3-year overall survival rate between the microwave ablation group and the operation group(P > 0.05).Conclusions:When the diameter of the tumors was less than 3 cm,the effect of microwave ablation was similar to that of the operation group,and there was no significant difference in the prognosis between the two groups.The tumor diameter was 3-5 cm.The late prognosis(recurrence rate and tumor-free survival rate)of the surgical group was better than that of the microwave ablation group.Compared with surgical resection,CT-guided microwave ablation of hepatocellular carcinoma has the advantages of less trauma,fewer complications,faster recovery and less risk.It has important application value in the treatment of hepatocellular carcinoma. |
