ObjectiveExplore the clinical characteristics and monitoring points of patients with peptic ulcer hemorrhage caused by Non-steroidal anti-inflammatory drugs,so as to provide useful reference for clinical practice.MethodsCollect the clinical data of 205 patients with non-variceal upper gastrointestinal bleeding admitted to The People's Hospital Of Anyang City from January 2018 to June 2018.According to the history of non-steroidal anti-inflammatory drugs taken in the first two weeks before admission,the patients were divided into non-steroidal antiinflammatory drugs group and non-steroidal anti-inflammatory drugs group,of which 59 were non-steroidal anti-inflammatory drugs group and 146 were non-steroidal antiinflammatory drugs group.The clinical characteristics and monitoring points of nonsteroidal anti-inflammatory drugs-related peptic ulcer hemorrhage were discussed by comparing and analyzing the age,sex,past history of gastric diseases,clinical symptoms of digestive tract and medication status of the two groups.Results1.Analysis of the non-steroidal anti-inflammatory drug-related peptic ulcer bleeding subjects collected in this study,the specific patient situation is as follows: the number of patients in the final screening in this clinical study was 205,including 150 male patients,accounting for the overall The ratio was 73.2%,and the number of female patients was 55.The overall proportion was 26.8%.2.From the gender perspective,in the data of patients who took non-steroidal anti-inflammatory drugs,the number of male patients was 36,and the number of female patients was 23;this time,the non-steroidal anti-inflammatory was not taken.According to the patient data information of the drug,the number of male patients was 114,the number of female patients was 32,the female patients in the non-steroidal anti-inflammatory group and the non-steroidal anti-inflammatory drug group.The proportion was significantly different(P=0.013).3.Among patients who received non-steroidal anti-inflammatory drugs,the number of patients aged ?60 years was 32,and the number of patients <60 years old was 27 patients.Among patients who did not take non-steroidal anti-inflammatory drugs,the number of patients aged ? 60 years was 41,and the number of patients < 60 years old was 105.There was a significant difference in the proportion of patients aged ? 60 years in both groups(P = 0.000).4.Gastrointestinal symptoms are one of the main factors in the analysis of clinical characteristics of non-steroidal anti-inflammatory drugs related to peptic ulcer bleeding.In terms of gastrointestinal symptoms,among the patients with peptic ulcer caused by the non-steroidal anti-inflammatory group,19.5%(40 cases)had different degrees of digestive tract symptoms.Among the subjects who did not take the non-steroidal anti-inflammatory group,there were 48.7% of the patients(100 cases)with symptoms of upper abdominal discomfort or upper abdominal pain,vomiting,and other non-steroidal anti-inflammatory drugs.There was a significant difference in the proportion of patients with gastrointestinal symptoms between the two groups who did not take the non-steroidal anti-inflammatory drug group(P<0.01).5.Past medical history is an important factor in the analysis of clinical characteristics of non-steroidal anti-inflammatory drugs related to peptic ulcer bleeding.In the comparison of the past medical history between the two groups of non-steroidal anti-inflammatory drugs and non-steroidal anti-inflammatory drugs,there were 4 cases with a history of peptic ulcer.There were 43 patients with a history of peptic ulcer in the non-steroidal anti-inflammatory group.There was a statistically significant difference in the proportion of patients with previous history of ulcer disease between the two groups(P<0.01).In the non-steroidal anti-inflammatory group,there were 17 patients with a history of ulcer bleeding and 33 patients with a history of cerebrovascular disease.The non-steroidal anti-inflammatory group had 15 patients with a history of ulcer bleeding;28 patients with a history of cardiovascular and cerebrovascular disease.6.Endoscopic ulcer characteristics have important reference value for understanding the clinical characteristics of non-steroidal anti-inflammatory drugs related to peptic ulcer bleeding.In the non-steroidal anti-inflammatory group,there were 10 patients with ulcer diameter ? 2.0cm,49 patients with ulcer diameter <2.0cm,and 21 patients with non-steroidal anti-inflammatory drugs with ulcer diameter ?2.0cm.125 patients with ulcer diameter <2.0cm.There was no statistically significant difference in the proportion of ulcer diameter between the two groups(P=0.642).7.Helicobacter pylori is an important data for analyzing and investigating the clinical features of non-steroidal anti-inflammatory drugs associated with peptic ulcer bleeding.Specifically,among the patients in the non-steroidal anti-inflammatory group,the number of patients with Helicobacter pylori infection was 18,and in the number of patients not taking the non-steroidal anti-inflammatory group,Helicobacter pylori infection was positive.The number of patients was 93.The proportion of Helicobacter pylori infection between the two groups was statistically significant(P = 0.000).8.Reasons for taking medication for patients taking non-steroidal anti-inflammatory drugs:(1)Cerebrovascular disease.The number of such patients was 29,accounting for 49.2% of the researchers;(2)respiratory infections,the number of such patients was 16 cases,accounting for 27.1%;(3)joint diseases.The number of such patients was 11 cases,accounting for 18.6%;(4)the number of patients with diseases such as toothache,dysmenorrhea,and soft tissue injury was 3,and the proportion of researchers was 5.1%.Conclusion1.The incidence of non-steroidal anti-inflammatory drugs-related upper gastrointestinal bleeding is increasing year by year.non-steroidal anti-inflammatory drugs is an important cause of upper gastrointestinal bleeding.2.Age,past history of cardiovascular and cerebrovascular diseases,past history of peptic ulcer and long-term medication were risk factors for non-steroidal anti-inflammatory drugs-related upper gastrointestinal bleeding.3.The clinical manifestations of non-steroidal anti-inflammatory drugs-related upper gastrointestinal hemorrhage are characterized by occult onset,no obvious abdominal pain and large amount of bleeding.
