Study On The Change Trend Of Liver Function In Patients With Cirrhotic Portal Hypertension Treated With TIPS

Objective: To investigate the change trend of liver function in patients with cirrhotic portal hypertension before and after transjugular intrahepatic portosystemic shunt(TIPS).Method: A retrospective study including 60 patients with cirrhotic portal hypertension who underwent TIPS between October 2016 and June 2018,was conducted.Liver function of patients before and after surgery(1 week after surgery,1 month after surgery,3 months and 6 months after surgery)were collected.And then the patient's Child Pugh score,end-stage liver disease model(MELD)score,and albumin-bilirubin(ALBI)score were calculated.Statistical methods: t-test was used for measurement data.Liver function(transaminase,bilirubin,albumin)at five time points was analyzed by repeated measurement analysis of variance of self control.And the count data were analyzed Chi-square test.Results:(1)The variance analysis of repeated measurements of lg(ALT),lg(AST)and lg(GGT)at five time points before and after surgery showed that the F values were 22.41,8.31 and 6.27,respectively,P < 0.05,with statistical significance.(2)The performance of 60 patients before and after TIPS was AST/ALT>1.(3)The variance analysis of repeated measurements of lg(TBIL),lg(DBIL)and lg(IBIL)at five time points before and after surgery showed that the F values were 16.89,19.64 and 11.19,respectively,P < 0.05,with statistical significance.(4)There was a slight decrease in albumin after surgery,which improved rapidly.The difference was statistically significant(F = 16.33,P < 0.05).(5)There were no significant differences in Child Pugh,MELD and ALBI scores between 6 months after surgery and before surgery(P values were 0.13,0.07 and 0.79,respectively).Conclusion:(1)TIPS can cause temporary and slight damage to liver function,but the effect on bilirubin is longer than that on transaminase.(2)Patients with decompensated cirrhosis may always have higher AST levels than ALT levels measured at the same time.(3)TIPS may improve the function of liver synthesis(synthesis of serumalbumin).(4)The liver reserve function was not affected in 6 months after TIPS.
