Objective:To study the psycho-acoustic and clinical characteristics of idiopathic tinnitus,To explore the impact of tinnitus frequency,tinnitus loudness on the hearing loss in order to improve clinic therapy for tinnitus.Method:In this study,110 patients with idiopathic tinnitus were grouped according to the observation indicators,and the patient's medical history was collected,including gender,age,duration,etc.;basic examination of the ear,including pure tone threshold measurement,acoustic impedance,etc.;and psychoacoustics Detection,including the degree of hearing loss,tinnitus sites(including tinnitus frequency and loudness),residual inhibition test,multi-complex acoustic therapy(TMIST)effect and THI score,etc.The psychoacoustic and clinical features of 9 tinnitus patients,compared with the patient's tinnitus There was a correlation between the frequency and the frequency of severe hearing loss,the ringing loudness and the degree of hearing loss,and whether there was a correlation between the residual inhibition result and the tinnitus effect,and whether there was a correlation between the tinnitus disability scale and the course of the disease.Result:In this study,the proportion of the age group was the highest in the group of 20 to60 years old,accounting for 95.45%(105/110),and the number of workers under 20 years old and 60 years old was significantly reduced.48 workers(44%)were mainly engaged in manual labor.59 workers(54%)were mainly engaged in mental work,and 3(2%)were unemployed.There were 67 cases(61%)in college and above college education;the proportion of single ear tinnitus in this study was significantly higher than binaural tinnitus.Monotonous 84 cases(76.4%)and continued cases of 88 cases(80%);25 cases(22.7%)with normal pure tone audiometry and 85 cases(77.3%)with hearing loss,measured fine voiced audiometry 2 cases(2%)with normal hearing loss and 108 cases(98%)with hearing loss,the two standards were consistent,and the positive rate of hearing loss after remarkable detection was significantly improved;the frequency of tinnitus locus and the maximum frequency of hearing loss There were 67cases(60.9%)with complete agreement;the frequency of tinnitus was 36 cases before or after the maximum frequency of hearing loss on the hearing curve(32.7%),Pearson correlation analysis The correlation between the two is found(r = 0.989,P <0.05);the results of ear-ring loudness matching are mostly concentrated between 40-60dBSL(37.3%),and Pearson correlation analysis shows that it has a correlation with the degree of hearing loss(r = 0.932,P <0.05);In this study,multi-complexed sound was used to treat tinnitus,and the overall effective rate reached 67.3%.There was a significant correlation between the results of residual inhibition test and the evaluation of multi-composite acoustic therapy(r =0.438,P <0.05);residual inhibition test knot There were statistically significant differences in THI scores between the two groups,positive and negative,and negative(r =-2.464,P < 0.05).There were 83 patients with a disease duration of less than 1 year,accounting for 75.5%,and the disease duration was within3 months.Of the 39 patients,35.5%,there was no correlation between THI and duration of disease(r =-0.047,P > 0.05).Conclusion:There was a correlation between tinnitus pitch and hearing loss;Residual inhibition may indicate the effectiveness of the sound therapy and may affect thepatient's THI score.Full Precision Test can provide clinical reference for early detection of hearing loss in tinnitus patients.
