Objective: To investigate the diagnosis and treatment of patients with acute and chronic pancreatitis who are the first symptom of pancreatic cancer;Methods: Retrospective analysis of 16 cases of pancreatic cancer patients with acute and chronic pancreatitis as the first symptom of biliary and pancreatic surgery admitted to the Provincial Hospital of Anhui Medical University from January 2008 to June 2016.Laboratory data such as hematuria amylase,biochemical,tumor markers,abdominal ultrasound,CT plain scan + enhancement,MRCP examination and other imaging data were collected at the time of admission.All specimens were sent to pathological examination and patients were followed up to obtain relevant diagnosis and treatment conclusions.RESULTS: Of the 16 patients with pancreatic cancer,8 had a history of acute pancreatitis,8 had a history of chronic pancreatitis,and the preoperative CA199 level was(246.78±626.21).Imaging findings suggest a high likelihood of pancreatic cancer,of which 3 The tumors invaded the mesenteric arteriovenous,2 cases showed tumor invasion of the spleen arteriovenous,1 case invaded the common hepatic artery and celiac trunk;14 cases underwent surgical treatment,3 cases underwent pancreaticoduodenectomy and 4 cases underwent pancreatic body Tail + splenectomy,6 cases of palliative cholangioenterostomy,1 case of laparotomy + anhydrous alcohol celiac ganglion injection;intraoperative tumor size was 2x3 cm ~ 10x8 cm,including 10 cases of pancreatic head,pancreas 4 cases of body tail;all patients with pathological examination results showed pancreatic cancer;12 cases were followed up,the average survival time was 4.8 months.Conclusion: The clinical manifestations of pancreatic cancer patients with acute and chronic pancreatitis as the first manifestation are non-specific.It is necessary to combine multiple examination methods to improve the accuracy of diagnosis.Surgery is currently the most important treatment,but the prognosis is poor and the survival time is not good.Therefore,patients with acute and chronic pancreatitis who have abnormalities in the relevant examinations should be alert to the possibility of developing pancreatic cancer.
