Objective:pituitary tumor is a common benign intracranial tumor,and its clinical manifestations are symptoms of tumor compression and endocrine abnormalities.However,with the further research on pituitary adenoma in recent years,it is found that pituitary adenoma may also lead to cognitive impairment.Among these cognitive impairment,emotional dysfunction is particularly obvious in patients with pituitary tumor,which seriously affects the quality of life of patients.Through the study on the ability to understand and regulate the emotion of patients with pituitary tumor,this study aims to understand the change of the emotion ability of patients with pituitary tumor and the effect of transnasal discectomy on the emotion ability of patients with pituitary tumor,and to explore the internal neural mechanism of this change.Methods:this study included 28 patients with pituitary adenoma who were clinically diagnosed and finally underwent rhinosaucer tumor resection,and recruited 28 healthy control subjects with matching gender and age.Based on event-related potential technology,50 negative,neutral and positive stimulation images in the international emotional picture system database were selected,respectively.The emotional image stimulation task was performed on 28 patients with pituitary tumor before and 3-6 months after surgery,and the same task was repeated on 28 healthy control subjects.NeuroScan 64 lead Ag/AgCl electrode cap was used to collect eeg data.The obtained eeg data were analyzed offline to obtain ERPs images and corresponding eeg topography of the subjects.Select N170 and LPP for target components to F3,Fz,F4,C3,C4,Cz,P3,Pz,P4 electrode for the target position,the pituitary tumor preoperative and postoperative pituitary tumor and healthy control subjects N170 and ingredient of LPP between average amplitude of groups and group interaction and ANOVA analysis of variance,P<0.05 for statistically significant standard,P<0.01 for significant statistical standards.Results:significant N170 and LPP components were extracted from all subjects under the stimulation of different types of images.ANOVA analysis was performed on the average amplitude of the two target components:N170 components:the N170 amplitude(-1.58 V)of the healthy control group was greater than that of the pituitary tumor before and after surgery(-0.98 V,-0.29 V),F(2,81)=26.86,p<0.01.In the healthy control group and the pituitary tumor postoperative group,the emotional(negative,positive)picture stimulation(-1.80 V,-0.38 V)caused a larger amplitude than the neutral(-1.04 V,-0.10,p<0.05)s and the negative picture stimulation induced the largest amplitude(-2.18 V,-0.69 V,p<0.01),while the pituitary tumor preoperative group showed no such statistical difference.In all subjects,the occipital electrodes displayed by Pz exhibited maximum N170 amplitude(-2.29 V,-1.46 V,-0.83 V,p<0.01).LPP composition:in the healthy control group and the pituitary tumor postoperative group,the emotional(negative,positive)picture stimulation(1.30 V,1.20 V)caused a larger amplitude than the neutral(0.47 V,0.74 V,p<0.05),while the pituitary tumor preoperative group showed no such statistical difference.Compared with the pituitary tumor group,the emotional images induced larger LPP in the pituitary tumor group(1.20 V)compared with the preoperative group(0.98 V).In the healthy control group and the preoperative pituitary tumor group,the occipital electrode displayed by Pz showed the maximum LPP amplitude(2.06 V,1.25 V,p<0.01),and the left(F3,C3,Z3)electrode recorded larger LPP than the right(F4,C4,Z4).Conclusion:pituitary adenoma patients have abnormal ability of emotion understanding and emotion regulation.Transnasal disc pituitary tumor resection can partially improve the patient's ability to understand and regulate emotions,but can not make it return to the normal level.Pituitary tumors,as well as transnasal discectomy,also cause changes in the emotional areas of the brain,and their underlying neural mechanisms deserve further investigation.
