| The accessibility of drugs is directly related to whether citizens can obtain the corresponding drug treatment when they are ill.It is directly related to citizens’ right to life and health,directly reflecting the public health level of a region or country,and the price of drugs is the basic factor.Only when the national essential medicine system is successfully implemented and implemented smoothly in public hospitals,retail pharmacies and e-commerce platforms,can we better guarantee the fairness and affordability of basic medical care in China by 2020.Objective:This paper intends to understand the differences between essential drugs in public hospitals,retail markets and pharmaceutical e-commerce through the investigation and analysis of the market of essential drugs and describe the current situation of the retail market of essential drugs.The relevant influencing factors and laws affecting the supply and demand of essential drugs in the three fields were analyzed through the change of the price level of "different drugs" in different channels and data support was provided for the supply and management of drugs in the medical reform and the formulation of relevant departmental policies.Methods:Based on the literature research method and the field investigation method,this paper refers to the WHO/HAI standardization method.The quantitative data is established database in Excel,The statistical analysis of the common basic drug prices of 55 kinds of common medicines,the same specifications and the same dosage forms in public hospitals,retail markets and e-commerce platforms was analyzed with SPSS24.0 software.The Kruskal-Wallis symbol rank sum test was used to test the significance of the retail prices of the same essential drugs in all three channels.Qualitative data are processed by classification,induction,summarization and refinement.Results:1.Among the 55 kinds of basic medicines investigated,the retail prices of 45 kinds of western medicines in public hospitals,31 kinds of retail drugstores and 33 kinds of pharmaceutical e-commerce are available.The main retail prices of these three channels are mainly distributed in the price range of 0-20 yuan and 21-40 yuan.In the comparison between the retail prices of public hospitals and retail pharmacies,28.00% of the public hospitals of the original western medicine were higher than or equal to the retail market,22.86% of the public hospitals of western medicine generic drugs were higher than or equal to the retail market,and50.00% of the public hospitals of Chinese patent medicine were higher than or equal to the retail market.In the comparison between the retail prices of public hospitals and e-commerce platforms,62.96% of the public hospitals of western medicine original research have unified bidding purchase prices higher than or equal to the e-commerce platform,41.03% of public hospitals of western medicine generic drugs have unified bidding purchase prices higher than or equal to the ecommerce platform,and 30.00% of public hospitals of Chinese patent medicine have unified bidding purchase prices higher than or equal to the e-commerce platform.The retail prices of some essential drugs in public hospitals are not only higher than those in retail pharmacies and pharmaceutical e-commerce,but also vary greatly in individual drugs.2.Among the essential drugs investigated,there are 24 Western medicine essential drugs with international reference prices.The median price of the original drug in public hospitals,retail markets,and e-commerce platforms is higher than the international reference price.Pharmaceutical hospitals(8 types),retail pharmacies(12 types),and medical e-commerce(12 types)with a median price of more than 10 times the international reference price;the median price of some generic drugs is lower than the international reference price,generic drugs Pharmaceutical hospitals(5 types),retail pharmacies(9 types),and e-commerce platforms(5 types)with a median price exceeding 10 times the international reference price.3.The affordability of generic drugs is significantly lower than that of the original drug.In public hospitals,retail pharmacies and e-commerce platforms,there are 3 kinds,3 kinds and 2 kinds of affordable costs for the original research drugs,which are equivalent to 10 days of minimum wage.Three types of distribution channels: generic drugs and proprietary Chinese medicines all cost less than the minimum wage of 10 days4.There was no significant difference in the affordability of primary developed drugs(all P > 0.05)in public hospitals,retail pharmacies or medical e-commerce.There was no significant difference in the affordability of generic drugs for different diseases(all P > 0.05)in public hospitals,retail pharmacies or pharmaceutical e-commerce.Conclusions:1.Most basic drugs in public hospitals are lower than those in retail pharmacies and e-commerce platforms,but the retail prices of some basic drugs in public hospitals are higher than or equal to those in retail pharmacies and pharmaceutical e-commerce.2.There was no statistically significant difference in the median price ratio and affordability among public hospitals,retail pharmacies and e-commerce platforms(all P>0.05),while the unified bidding purchase price of public hospitals was the government’s regulation or control of drug prices to control the growth of medical expenses,that is,the inflated price of essential drugs in public hospitals under investigation.3.No matter it is a public medical institution,a retail market or an e-commerce platform,the median price of the original pharmaceutical research unit of essential drugs is higher than the international reference price.Retail prices of some generic drugs vary greatly in different channels. |
