Objective:With the method of retrospective analysis,the risk factors,clinical features and drug treatment in patients with heart failure of preserved ejection fraction in combination with atrial fibrillation were investigated,providing a reference for the prediction and reduction of atrial fibrillation.Methods:Collecting 198(76 cases in males,122 cases in females)patients of heart failure of preserved ejection fraction from the Department of Cardiology,the First Clinical Hospital of Jilin University between January 2017 and December 2017.We divided them into two groups: the atrial fibrillation group included 98 cases(49.50%),the number of men was 39(39.80%),the number of women was 59(60.20%),mean age was 76.0(67.5,81.0).The controls included 100 cases(50.50%),the number of men was 37(37.00%),the number of women was 63(63.00%),mean age was 74.5(66.0,81.0).The general dates,other concomitant diseases,serological results,echocardiographic parameters,drug treatment were compared.All comparisons were carried by SPSS 22.0 and results were considered statistically significant at level of P<0.05.We used multiple logistic regression analysis for results that were counted statistically significant.Results:1.Comparison of general data between the two groups showed that the heart rates per minute of patients in the AF group were higher than that in the N-AF group(P=0.005).2.Alanine aminotransferase,aspartate aminotransferase,triglyceride,total cholesterol and low density lipoprotein in AF group were higher than those in N-AF group(P <0.05).3.The left atrial diameter and right ventricular end-diastolic diameter of patients in the AF group were higher than those in the N-AF group,the percentage of patients with pulmonary hypertension was higher than that in the N-AF group(P<0.05).4.There were no statistically significant differences in the percentages of patients with hypertension,diabetes,chronic obstructive pulmonary disease and renal insufficiency between the two groups(P>0.05).5.There were no statistically significant differences in the percentages of patients treated with diuretics,beta blockers,ACEI/ARB,spironolactone,statins,and digoxin between the two groups(P >0.05).6.We used multiple logistic regression analysis for results that were counted statistically significant,suggesting that every 1mm increased in LAD,the risk of atrial fibrillation increased 1.117 times(OR=1.117;P < 0.001).Conclusion:After multiple logistic regression analysis of the clinical datas that were counted statistically significant,it is found that the increase of left atrial diameter caused by the change of left atrial structure is an important risk factor for atrial fibrillation in HFp EF patients.Monitoring the change of the inner diameter of the left atrium and early detection of atrial fibrillation have important clinical significance for delaying the progression of HFp EF and reducing the occurrence of atrial fibrillation stroke.
