Objective:With the aging of the population,the prevalence of type 2 diabetes mellitus and cognitive impairment is increasing.Many studies have shown that diabetes is an independent risk factor for cognitive impairment.Diabetic patients are more likely to have cognitive impairment than non-diabetic patients,and the risk of dementia is 50% higher than non-diabetic patients.And diabetes can increase the rate of progression to dementia by 1.5-3 times in patients with mild cognitive impairment.Therefore,exploring the risk factors of diabetic cognitive impairment is of great significance in preventing diabetic cognitive impairment and delaying the progress of diabetic cognitive impairment.Method:In this study,120 patients with type 2 diabetes mellitus aged 50-70 years old hospitalized in the Department of Endocrinology of the First Hospital of Jilin University from October 2017 to September 2018 were retrieved from the electronic database of in-patient medical records of the First Hospital of Jilin University.According to the MMSE scale and MoCA scale,the cognitive function of subjects was assessed and grouped.The demographic characteristics,lifestyle,clinical characteristics of diabetes mellitus and various laboratory indicators of all subjects were sorted out,classified and counted.The cross-sectional analysis of the related influencing factors of elderly patients with type 2 diabetes mellitus was conducted.Results:1.The independent risk factors of cognitive impairment in type 2 diabetes mellitus were age,education level,course of diabetes,FBG,HbA1 c,TC,TG and 24-hour urinary protein.2.There was a significant difference between the degree of fundus retinopathy and cognitive dysfunction in type 2 diabetes mellitus(P < 0.05).3.There was significant difference between the degree of macroangiopathy and cognitive impairment in type 2 diabetes mellitus(P < 0.05).4.Metabonomic analysis of cognitive impairment in type 2 diabetes mellitus: The results showed that there were significant differences in Gln,Glu,Phe,Tyr,Pro and Hcy among the patients with cognitive impairment in type 2 diabetes mellitus(P < 0.05).Glu,Phe,Tyr,Pro and Hcy all showed an upward trend with the aggravation of cognitive impairment,while Gln showed a downward trend with the aggravation of cognitive impairment.Potential.ROC curve area analysis of six amino acids showed that there were statistical differences among Glu,Gln,Phe and Pro.Conclusions:1.The independent risk factors of cognitive impairment in type 2 diabetes mellitus were age,education level,course of diabetes,FBG,HbA1 c,TC,TG and 24-hour urinary protein.2.The degree of cognitive impairment in type 2 diabetes mellitus is correlated with the severity of macrovascular and microvascular lesions.3.Glu,Gln,Pro and Phe can be used as diagnostic indicators of cognitive impairment in patients with type 2 diabetes mellitus.The combined diagnosis of glutamate and glutamine is more meaningful for the early diagnosis of type 2 diabetes mellitus with cognitive impairment.
