Objective:To study the expression levels of p16ink4 a and Ki67 in colorectal cancer tissues and the preoperative CEA levels of colorectal cancer patients,and to analyze the relationship between the expression levels of p16ink4 a,Ki67 and CEA and the clinicopathological characteristics,so as to provide new ideas for the diagnosis and treatment of colorectal cancer.Materials and methods:Sixty patients who underwent Radical surgery for colorectal cancer in gastrointestinal and colorectal surgery department of china-japan union hospital of jilin university from June 2015 to December 2016 were collected through electronic medical record system.No intervention was performed before the surgery,and postoperative pathological diagnosis was made.Among them,there were 31 males and 29 females,ranging from 30 to 91 years old,with an average age(59.3±10.9).There were 24 cases with lymph node metastasis,36 cases without lymph node metastasis,22 cases with tumor thrombectomy,38 cases without tumor thrombectomy,47 cases with highly and moderately differentiated adenocarcinoma,and 13 cases with poorly differentiated adenocarcinoma.Venous blood of the patients was extracted preoperatively to detect CEA level,and 60 cases of carcinoma tissue wax mass were taken,and 30 cases of paracancerous tissue.In addition,5 randomly selected colorectal cancer tissues and 5 adjacent tissues were tested by Western blot.Monoclonal antibodies p16ink4 a and Ki67 were detected by immunohistochemistry.Chi-square test or Fisher test was used to analyze the relationship between the expression of p16ink4 a,Ki67 and CEA and the clinicopathological characteristics.P <0.05 was considered statistically significant.Results:1.P16ink4 a was positively expressed in 32 of the 60 colorectal cancer tissues and negatively expressed in 28 of them,with a positive rate of 53.3%.Among the 30 paracancer tissues,23 showed positive expression,7 showed negative expression,and the positive rate was 76.7%.The difference was statistically significant(c2= 4.582,P <0.05).The positive rate of p16ink4 a in patients with highly and moderately differentiated adenocarcinoma was significantly higher than that in patients with poorly differentiated adenocarcinoma,and the difference was statistically significant(P < 0.05).? + ? period in patients with p16 positive rate is higher than ? + ? period of patients,the difference is statistically significant(P < 0.05).There was no correlation between positive expression of p16ink4 a and age,gender,tumor size,presence of tumor thrombus or lymph node metastasis(P > 0.05).2.Ki67 was highly expressed in 38 of the 60 colorectal cancer tissues,and was poorly expressed in 22 cases,with a high expression rate of 63.3%.Among the 30 paracancer tissues,6 cases showed high expression,24 cases showed low expression,and the high expression rate was 20%.The difference was statistically significant(c2=15.030,P < 0.001).Ki67 in ? + ? expression levels were significantly higher than those of patients with ? + ? period,the difference was statistically significant(P <0.05).The expression level of Ki67 in patients with lymph node metastasis was significantly higher than that in patients without lymph node metastasis,and the difference was statistically significant(P < 0.05).The expression level of Ki67 was not correlated with age,gender,differentiation type,tumor size,and presence of tumor thrombus(P > 0.05).3.Relationship between preoperative CEA level and clinicopathological characteristics in patients with colorectal cancer.The CEA level of patients with low-differentiated adenocarcinoma was significantly higher than that of patients withhigh-differentiated adenocarcinoma,and the difference was statistically significant(P< 0.05).Among patients with lymph node metastasis,patients with increased CEA level were significantly more than those without lymph node metastasis,and the difference was statistically significant(P < 0.05).? + ? period in patients with elevated serum CEA levels were significantly more than ? + ? period,the difference is statistically significant(P < 0.05).There was no correlation between patients' CEA level and age,gender,tumor size and presence of tumor thrombus(P > 0.05).4.Western blot results showed that the relative expression of p16ink4 a in colorectal cancer tissues was lower than that in paracancer tissues,and the relative expression of Ki67 in colorectal cancer tissues was higher than that in paracancer tissues.Conclusion:1.The expression level of p16ink4 a in CRC tissues was lower than that in the corresponding paracancer tissues,and its positive expression was correlated with the differentiation type and TNM stage of CRC.2.The expression level of Ki67 in CRC tissues was higher than its corresponding paracancer tissues,and its high expression was related to the TNM stage of CRC and whether there was lymph node metastasis.3.CEA level was correlated with CRC differentiation type,lymph node metastasis and TNM stage.4.There is a correlation between p16ink4 a,Ki67 and CEA and the clinicopathological characteristics of CRC patients.However,whether the combined detection of p16ink4 a,Ki67 and CEA is helpful for the prognosis evaluation of CRC still needs to be confirmed by further studies.
